Person Centered Services provides Care Coordination for over 17,000 people with intellectual and/or developmental disabilities in 18 counties, reaching throughout the Western New York, Finger Lakes, and Southern Tier regions. Our job is to help individuals and families navigate a very complex system.
At the heart of Care Coordination are connections and referrals. Care Coordinators work with individuals, families, community providers and other organizations to make sure the dots are connected between all aspects of a person’s care.
