It honestly depends from patient to patient. Some patients require little to no help and you just have a to be there to keep them company, watch tv with them, read to them etc. and some patients require you to do everything for them. Toilet them, bathe them, cook for them and feed them, lift them/transport them. Also it depends if you are working in a nursing home or doing at home and how much time the patient required. Are they just a few hours a day care? Or 24 hours a day care.
