Day Program/Residential LPN
QUALIFICATIONS: Currently licensed as a Licensed Practical Nurse in the State of Massachusetts. Possess a valid driver’s license.
SUPERVISED BY: Healthcare Coordinator
SUMMARY: Overall, the Licensed Practical Nurse is responsible for monitoring and maintaining the individual’s medical status/issues in programs, and health maintenance in conjunction with the individual’s Health Care Professional.
General responsibilities
- Maintain compliance with all regulatory standards and documentation requirements of MassHealth, Department of Developmental Disabilities and other monitoring bodies
- To be informed of and respect the Human and Legal rights of all individuals.
- To maintain the confidentiality of any and all individuals at all times.
- Upon request, to initiate, maintain and ensure ongoing communication between Bass River, Families, Residences, Service Coordinators/QMRP, Consultants and/or any individual/agency.
- Assist in the maintenance of CARF accreditation status.
- Upon request, to participate in the screening/evaluation of individuals being considered for acceptance (admission) to Bass River.
- To attend in-services, workshops, conferences as required/requested by Bass River, Inc.
- Report relevant staff/individual issues to Program Director.
- To participate in the development and implementation of program content based upon individual needs and program focus.
- To provide direct care coverage as necessary.
NURSING RESPONSIBILITIES:
- To work cooperatively with the Health Care Coordinator (HCC) and other nursing staff to Provide nursing coverage to Bass River Programs as requested.
- To accurately administer medication to individuals as ordered by their HCP and in accordance with MAP regulations.
- Set up and monitor tube feedings according to HCP orders.
- To participate in individuals medical appointments as requested.
- Provide routine and emergency medical services to individuals as needed or according to HCP orders.
- Maintain up to date medical information for individuals and communicate with the Health Care Coordinator and Program Directors about individual status as indicated.
- To provide training and ongoing supervision to individuals and staff as delegated by the registered nurse (RN).
- To implement and when requested to provide supervision regarding the implementation of infection control and Universal Precautions policies.
- To support the agency in the maintenance of equipment and the program area, especially in regard to health and safety issues
- In collaboration with the HCC or RN, interpret and communicate medical information as requested.
- To report program/clinical needs to the Director in a timely and orderly fashion and to follow through as requested.
- Driving individuals or agency vehicles as directed by supervisor as required.
