POSITION SUMMARY: 

Under general supervision, the LPN provides patient care services in accordance with scope of practice as defined by state law and established policies and organizational procedures.  Provides patient care by assisting with the collection of patient health data to be used to complete a physical assessment, administering treatment and medications and performing diagnostic tests ordered.  Accurately records patient medical history, performs medication reconciliation, provides patient education and facilitates care coordination and continuity of care acting as a liaison with other members of patient’s medical team.

 

QUALIFICATIONS:

Education

  • See “License” below.

Experience

  • A minimum of 6 months prior experience with Primary Care LPN direct patient care work required.
  • Experience with Athena EMR preferred.

Certification, License, Registration 

  • LPN License in good standing with SCLLR required.
  • Current BLS certification required.

Special Skills

  • Bilingual fluency in Spanish and English preferred. 
  • Strong customer service focus required.
  • Effective verbal and written communication skills required.
  • Organized, flexible and able to manage competing priorities required. 
  • Evidence of sound clinical judgment and resourcefulness in problem solving required.
  • Able to receive and follow through with delegated tasks and accountability required.
  • High level computer skills, including but not limited to Microsoft Office software required. 
  • Ability to add, subtract, multiply, divide, compute rate, ratio, and percent required.

  • Regularly lift, exert, move up to 25 pounds and occasionally lift and/or move up to 50 pounds required.

  • Specific vision abilities required including close vision, color vision, depth perception, peripheral vision and ability to adjust focus.
  • Generally, a Light/Medium physical activity level working with ambulatory patients with occasional need to assist patients physically.

General Working Conditions 

  • Routine exposure to bloodborne and airborne pathogens and sick visit patients in a healthcare setting.
  • Works generally indoors and clinic type environments but requires motivation to other areas of the building.
  • The noise level in the work environment is usually moderate. 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

To perform this job successfully, an  individual must be able to perform each essential duty without accommodation or with accommodation without undue hardship to Careteam Plus.

  • Performs occupational and health screenings ordered by Providers.  
  • Verifies patient information by interviewing patient, recording medical history, confirming reason(s) for visit. 
  • Prepares patients for examination by “rooming”, performing preliminary physical tests, taking vitals and taking and recording patient history summary. 
  • Collaborates with providers to arrange for medical treatment and complete referrals as ordered 
  • Recognizes equipment repair needs and brings it to the attention of the Clinical Services leader. 
  • Conducts appropriate CLIA Waived laboratory tests and quality control procedures as ordered by provider. 
  • Documents accurately all elements of patient care including but not limited to, communication, treatments, immunizations, medications administered, discharge instructions provided, and follow up care arranged.
  • Collaborates with provider to evaluate health education/promotion needs of patients and provides necessary training and instruction.
  • Promotes and encourages patient portal enrolment to enhance communications and customer service.
  • Identifies emergency situations and provides care as directed by Provider.  
  • Administers medications and medical treatments competently as directed by provider. 
  • Maintains medical records using electronic health/medical record timely.  
  • Maintains and contributes to a clean and safe environment.  Follows and promotes infection control procedures. 
  • Maintains patient privacy and confidential patient information in accordance with established policies and procedures and HIPAA regulations at all times. 
  • Participates in follow up phone calls/contacts to meet patient needs daily. 
  • Assists with facilitating prior-authorizations and pre-certifications as needed. 
  • Provides services across the continuum of care by collaborating with other organizations, following up on no-show visits, tracking return to office orders, diagnostic imaging orders, laboratory orders, and referral orders to completion in accordance with established procedures. 
  • Completes pre-visit chart prep planning by reviewing patient care needs prior to patient visit according to established clinical guidelines. 
  • Participates in weekly point of care chart closures and huddle discussions.
  • Participate in Quality Improvement Program activities, CQI efforts, and applicable meetings. 
  • Effectively and efficiently utilizes EHR to assist organizational CQI efforts, clinical data validation, coordination with external providers, and completeness and accuracy of patient record.
  • Routine duties are performed with minimal supervision; standard practices or procedures allow LPN to proceed alone at routine work; occasional check of work while in progress; and work reviewed upon completion. 
  • Careteam staff work together as a team and help each other and patients/clients by performing other duties as assigned.
