Household Manager/Social Worker

Heartwood is seeking a full time Household Manager in our new Long-Term Care facility. Salary range: $60,070 - $75,000 annually.

In this role, you will serve as the resident's advocate to create and maintain an environment that achieves high quality care (physical, mental, emotional, social and spiritual) and helps to maintain their independence and dignity to highest degree possible.

  • Oversee the quality of life of the residents including privacy, dignity, meaningful activity, food enjoyment, autonomy, individuality, relationships, and other needs.
  • Ensure the resident's care plan incorporates the resident's social preferences and life history information.
  • Supervise the Resident Assistants and ensures they are engaged and participating in the activities of daily living with the residents.
  • Involved in Quality Assurance meetings and assists in establishing best practices to provide quality and safety outcomes for residents and staff.
  • Complete required MDS assessments and related documentation within the resident electronic charting software ensuring compliance with regulations and established best practices.
  • Develop, maintain and lead a team that achieves desired outcomes at the individual team member level as at the Household level.
  • Responsible for discharge planning and coordination for short stay residents, including referrals for services and resources needed to set discharging residents up for success.
  • Issue denials including Notice of Non-Coverage for Medicare, Medicare Advantage and MSHO plan.
  • Meets with new residents and/or their representatives for completion of admission agreements on day of admission.
  • Member of the leadership team that sets the strategic direction for the organization.

Qualifications:

  • Three or more years on the job experience in older adult services. Education, experience or licensure in geriatric social work, therapeutic recreation or gerontology preferred.
  • Demonstrated leadership/supervisory skills to develop, motivate, and lead staff and design work processes. Prior management experience preferred.
  • Competencies in quality management, resident and family support systems, community referral and networking, budget development, and fiscal responsibility.
  • Strong interpersonal and relationship skills, critical thinking, and problem-solving skills.
  • Successful completion of a state approved nursing assistant training program and current certification in the State of Minnesota or ability to become certified with six months of hire.
