Life Coach Case Manager

The City of Saint Paul’s Office of Neighborhood Safety is now hiring Life Coach Case Managers. In this role, you’ll provide ongoing case management support for Project PEACE and Familiar Faces—two vital programs working to build a safer, more connected community.

We currently have three full-time positions available: one certified vacancy supporting Project PEACE, and two provisional vacancies supporting the Familiar Faces initiative. These provisional positions are grant-funded. Funding is available through May 2026 with anticipation of refunding. 

Here’s what you can expect to do:
Life Coach Case Managers in the Project PEACE program are assigned to work with the young adults in the City who have been identified as being at very high risk of being involved in gun violence or individuals who are victims of gun violence and community violence and their families. Life Coaches work primarily with persons and families who have extensive criminal justice involvement; are members of groups; and have been shot before and/or have close friends who have been shot recently, individuals who are victims of violence.

Life Coach Case Managers in the Familiar Faces program are assigned to work with unhoused individuals in the City identified as individuals who are high utilizers of City emergency services and hospitals. Life Coaches work with individuals who may have substance use disorders, mental health challenges, and are experiencing unsheltered homelessness. 

Primary Responsibilities:

  • Provide in-person client connection with assigned clients to include assessing education, employment, housing, food, social/emotional, behavioral and basic necessity needs and gaps.
  • Assess challenges and barriers related to justice involvement and educational or vocational needs, aptitudes, and interests.
  • Apply for immediate and emergency services as necessary for family preservation.
  • Develop ongoing service plans for families and individuals, including providing community referrals, completing necessary paperwork, and documenting necessary service plans and case records.
  • Engage clients in ongoing treatment process through individual and group counseling, including establishing goals and objectives for improvement in identified areas through life coaching and assessing progress
  • Coordinate care and appointments for clients when necessary for physical, mental, and behavioral health as well as justice involvement and other social service appointments, conducts case management conferences with provider partners serving the individual and family

Provisional Employment: The Familiar Faces vacancies are open to anyone who meets the position requirements. Provisional employees are not certified employees, but are eligible to receive benefits on the 1st of the month following 30 days of employment.

Position Requirements

Successful candidates typically enter this role with six years of relevant education and work experience. That path to qualification would usually include:

A Bachelor’s Degree in Counseling, Public Safety, Public Health, Public Policy, Criminal Justice, Psychology, Political Science, Sociology, Social Work, or a closely related field, and;
 

Two years of work experience in individual and family crisis counseling, life coaching or closely related experience; preferably with a high-risk client population.

Substitution:

  • Relevant work experience in the above areas that would result in specialized knowledge comparable to the above degree can be substituted for education on a year-for-year basis.
  • An Associate degree in a field listed above will qualify for up to two years of work experience; therefore, candidates would need an additional four years of experience.
  • A Master’s degree in a field listed above will substitute for up to one year of the required work experience.
  • Additional certifications and training which could be counted for experience may be specified in the job posting.

Must possess and maintain a valid Minnesota Class D Driver's License or equivalent out-of-state driver's license.
