We have an excellent Full-Time opportunity for a dynamic Home Health Physical Therapist (PT) to care for our residents in the comfort of their own homes. Must be licensed in the State of Missouri as a physical therapist, have excellent communication and computer skills as well as reliable transportation.

You will:
- Assess the patient’s physical, communication and cognitive skills to develop a treatment plan
- Provide advice and support for family members
- Assess the home environments of the patient
- Educate and instruct on assistive devices and equipment
- Provide home exercise program
- Assist patients’ exercise for pain relief
- Review the patient’s progress and adapting the treatment accordingly
- Perform administrative tasks such as keeping records, writing reports, and making telephone calls
