Case Manager
Arrowhead West, Inc. Central Kansas Division headquartered in Wichita, KS, is seeking a detail oriented, compassionate candidate for a Case Management Position. Case Managers assist individuals with intellectual and developmental disabilities in identifying and coordinating the use of both paid services and natural supports available to enhance the person’s independence and community participation based on the person’s capabilities and preferences.
Responsibilities:
- Completes intake, transfer, discharge, and follow-up procedures for all individuals assigned to caseload
- Serves as an advocate for people on caseload and completes monthly reviews on development and monitoring of Individual Support Plan (ISP)
- Chairperson of the Person Centered Support Plan (PCSP) team (unless client has chosen another team leader) and coordinates the planning, development and implementation of the plan
- Works closely with Community Employment Manager, Program Manager, Residential Manager, Residential Coordinator, and other IDD service providers in developing client services and monitoring quality service delivery
- Communicates with client and/or their family/legal representative and ensures client services are provided and maintained
- Visits clients on caseload at program sites on a regular basis, provides feedback and guidance to direct care staff about implementation of PCSP on site
Requirements:
- Bachelor’s degree & six months experience in disability supports OR
- the requirements for the State of Kansas to include a minimum of 6 months experience in the field of human services AND either a Bachelor’s degree or additional experience in the DD field which may be substituted for the degree at the rate of 6 months of full time experience for each missing semester of college
- Must meet registration requirements to provide case management services through Kansas Department for Aging and Disability Services (KDADS)
- Valid driver's license
