JOB DESCRIPTION

JOB TITLE:              LPN

JOB FUNCTION:    Delivers training and/or health care to designated individuals.

REPORTS TO:         Director of Nursing/RN Nurse Trainer

LOCATION:             Assigned to Various Sites

JOB RESPONSIBILITIES

1. Actively support and further the Mission of The Arc.
2. Work in accordance with Illinois Administrative Rule 116 and in conjunction with other ARC personnel of The Arc to provide supportive assistance to individuals served by:
3. Administering prescribed medication and/or treatments and maintaining necessary documentation in accordance with Illinois Administrative Rule 116 and The Arc's policies and procedures.
4. Delivering instruction and/or health care on an individual and/or small group basis to designated individuals in accordance with The Arc's policies and procedures.
5. Assessing individuals and maintaining necessary data documentation in accordance with prescribed guidelines.
6. Participating in individual staffings and IDTs when appropriate.
7. Provide supervision and ongoing assistance in the personal and social development of designated individuals The Arc serves.
8. As requested, participate in conferences, seminars, meetings, and in-service training sessions.
9. Responsible for making recommendations to the supervisor to ensure the health and safety of individuals The Arc serves.
10. To perform all tasks and duties as assigned by the supervisor.
     

QUALIFICATIONS

1. LPN with graduation from an accredited nursing school.
2. LPN licensed in the State of Illinois, current and in good standing.
3. Valid driver’s license in good standing and vehicle with appropriate auto insurance required.  Must be approved by the Agency’s Auto insurance carrier for driving agency vehicles.
4. Demonstrated strength and flexibility to assist in lifting individuals or provide other individual-related care.
