Resources and Services Navigator

Join the team at State Hospital South as a Resources and Services Navigator. This position is primarily dedicated to assisting State Hospital Patients transitioning to outpatient care. You will join a team that is passionate about working with individuals with mental health conditions and serve in the critical role of discharge planning. You will have the opportunity to link patients with outpatient services, work with outpatient providers, be part of an interdisciplinary treatment team, and work in a challenging and exciting hospital environment.

State Hospital South was established in 1886 and is in beautiful Southeast Idaho, an area best known for Oregon Trail and Native American history, amazing fly fishing, towering mountain peaks with abundant wildlife and awe-inspiring geological sites. State Hospital South in Blackfoot provides psychiatric inpatient treatment and skilled nursing care for Idaho’s adult citizens with the most serious and persistent mental illnesses. The hospital works in partnership with families and communities to enable clients to return to community living. State Hospital South includes 110 psychiatric adult beds and 42 skilled nursing beds. It also maintains a statewide program to restore the competency of criminal justice patients. 

This position is not eligible for telework.

The Idaho Department of Health & Welfare is a drug-free workplace. EXAMPLE OF DUTIES:

  • As a member of treatment team, attend meetings to discuss patient discharge needs and services.
  • Assess each client's health, social, and emotional needs to determine the least restrictive housing and treatment provider options.
  • Use critical thinking skills and professional judgment to assess each patient's current benefit status and potential benefit application needs at the time of admission and prior to discharge.
  • Serve as case manager for community services and resources in regards to discharge and benefit planning.
  • Be able to work independently and collaboratively as a member of a cohesive team.
  • Be able to effectively communicate both verbally and in writing with patients and internal and external partners.
  • Experience in identifying individual and family strengths and services needs through skillful interviewing and discussion with patients.
  • Some working knowledge of benefits programs including Medicaid, Medicare, Veteran's benefits, and Social Security Programs and Entitlements.
  • Some experience with individuals with special needs, such as mental health, developmental disabilities, and substance use disorders.

MINIMUM QUALIFICATIONS:

You must possess all the minimum qualifications below to pass the exam for this position. Please make sure your resume or work history supports your meeting the minimum qualifications for this position. Failure to do this may disqualify you from being considered for this position. It is highly recommended to attach a one-page cover letter to your application to demonstrate how you meet the requirements below.

  • Good knowledge of human services principles and practices. This is typically gained by a bachelor’s degree or higher in a human services field (social work, education, nursing, psychology, counseling, sociology, etc.) OR at least three years of work experience as a human services professional (e.g. social worker, counselor, rehabilitation specialist, etc.) assisting individuals and families with accessing community resources including benefits and services.
  • Experience in case management. This is typically gained by at least one year of professional case management experience that included case plan development and monitoring.
