Title: Case Planner [BSW]
Credential requirement: BSW
Status: Full-time, Non-Exempt
Salary: $41,000-$46,000
Hire Date: Hiring immediately - start date as soon as possible
Case Planner Responsibilities:
- Must hold an LMSW or LPC or LMHC License
- Handle non-ACS Mandated Cases (clinical mental health services under IMW) and ACS Mandated Cases (preventive service)
- Responsible for providing prevention services to high-risk families
- Conduct comprehensive assessments of family strengths, needs, and goals
- Develop individualized service plans in collaboration with families, incorporating their input and preferences
- Provide ongoing case management support, including crisis intervention, advocacy, and referral to community resources
- Coordinate with school staff, community partners, and other service providers to ensure seamless delivery of services
- Monitor progress towards goals and adjust service plans as needed
- Maintain accurate and up-to-date case records in accordance with agency policies and regulatory requirements
- Participate in team meetings, case conferences, and trainings as required
- Provide support and guidance to less experienced staff as needed
- Collaborate with program leadership to evaluate program effectiveness and identify areas for improvement
- Caseloads may be manageable to ensure case planners can effectively deliver quality case management; to ensure clients receive support and professional development, and for families to receive quality services
Student and Family Support:
- Review student data and make referrals for appropriate mental health services.
- Conduct comprehensive intake assessments with students and families.
- Facilitate ongoing family counseling sessions.
- Develop individualized and family treatment plans, working collaboratively with families to achieve goals.
- Engage parents/guardians in treatment planning and progress monitoring.
- Develop discharge plans and conduct ongoing assessments to track student and family progress.
School Collaboration:
- Build strong relationships with school personnel to ensure program effectiveness.
- Provide support to teachers and administrators through consultation, brief interventions, and behavioral planning.
- Communicate and collaborate with school officials (principals, teachers) to address student needs.
- Educate parents and staff regarding resources and programs for children with special needs.
Clinical Expertise:
- Conduct family screenings and develop comprehensive treatment plans.
- Facilitate on-site conflict resolution and peer mediation for students.
- Maintain detailed progress notes to track student improvement.
Compliance and Documentation:
- Manage case files and complete necessary forms and reports in accordance with district, state, and federal regulations.
- Maintain accurate and up-to-date records
- Any related-duties/tasks as assigned by supervisor
Qualifications / Requirements:
- Master’s degree in Social Work (MSW), Counseling, Psychology, or a related field.
- Relevant licensure (LCSW, LPC, or LMHC) is required.
- Minimum of 2-5 years of experience in case management, family support, or mental health services, particularly with high-risk families.
- Experience working with school systems and knowledge of educational resources and support services for students.
- Experience conducting individual and group counseling sessions with students and families.
- Excellent communication, interpersonal, and organizational skills
- Proficiency in Google Suite and case management software preferred
Travel Requirement:
- Ability to travel within New York City public schools to perform job-based duties.
- Must be a New York City Resident with a valid Social Security Number - I’RAISE does not provide relocation assistance nor provide any visa sponsorship at this time.
