Service Coordinator - Transitional Living Community - 2 (TLC2) - $18.54/Hr            

Closing Date: 04/28/2025 

 

FT 40 Hr/wk Assists program participants with recovery and harm reduction goals. Supports program participants in obtaining and maintaining a healthy lifestyle. Promotes individual growth and life enhancement, as well as the development of valued social roles. Responsibilities include: providing services related to service participant’s IRPs, person-centered goal planning, medication cueing, crisis intervention, and daily living skills (such as making and keeping important appointments with healthcare providers, budgeting, menu planning, laundry, grocery shopping, social skill development, and providing support for ADL). Documentation of services in the Electronic Medical Records System (EMRS). CNA/HHA/PCA/PCT preferred. 

Worksite: Norwich  

Hours: Mon - Fri, 8:00 am - 4:00 pm 

 

Education and Experience: 

  • Bachelor’s degree in human services or equivalent work experience in the provision of mental health preferred
  • Work experience in the provision of mental health services preferred
  • Valid CT Driver’s License, vehicle registration, and insurance
  • Good driving record
