Brain Injury Case Manager

Case Manager- Enjoy a rewarding and challenging position with a nonprofit organization helping educate, train and provide employment opportunities for individuals with disabilities since 1955. We are currently seeking applicants for a full-time Brain Injury Case who will be responsible for the assessment, coordination, and provision of services to adults living with brain injuries and their families. Spends a minimum of 50% of time in direct contact with clients and their families providing supportive counseling, coordination of services, resource determination, monitoring of services, and team meetings. Also, be responsible for the development and marketing of case management services within the established service area using a model of community-based case management.

Monday - Friday 8:00 AM -4:30 PM with occasional evenings and weekends.

 

ESSENTIAL DUTIES/RESPONSIBILITIES: 

The following duties and responsibilities represent the essential functions of this job that an individual must be able to perform with or without a reasonable accommodation:

  • Establishes a solid therapeutic relationship with clients and their families which requires communicating effectively with clients, their families, other support systems, and professionals
  • Maintains personal contact with the client and/or family at least monthly unless otherwise requested by the client and/or their family
  • Completes a thorough client assessment to include assessing the client's strengths and weaknesses, family and community support systems, existing resources presently utilized by the client, and gaps in service requiring intervention within 45 days of opening a case; assessing assistive technology needs and seeks appropriate resources to meet needs; and obtaining appropriate releases, understand the laws regarding confidentiality, and upholding these laws
  • Completes all paperwork promptly and maintains documentation in accordance with agency and CARF standards
  • Supports clients in developing their own goals and plans based on the needs assessment, completes a Case Management Plan, and provides a written copy to the client
  • Identify, coordinate, and monitor goal-directed community services to meet the client's goals and objectives as outlined in their plans
  • Create and facilitate support teams to include referrals, scheduling of meetings, and delegation of duties
  • Effectively negotiate for services for clients in their communities in a professional manner
  • Maintain a caseload of 25-30 active clients and 5-10 clients in follow-along status; and provide education to clients, their families, and the community regarding brain injury and community resources
  • Demonstrate a commitment to person-centered services and the opportunity for client choice in the selection of service providers as documented in the case management plan and progress notes
  • Monitor services by outside providers and advocates on behalf of clients
  • Works collaboratively with Beacon House Program Manager and Director of Vocational Rehabilitation Services to develop program policy and procedures
  • Attends staff meetings, case review meetings, and staff training activities as required
  • Other duties as assigned
  • Local travel is required in a personal vehicle

 

QUALIFICATIONS, EDUCATION, & EXPERIENCE: 

The following qualifications represent the training, education, certifications, licensures, and/or work experience required to perform the essential duties and responsibilities successfully:

  • Degree in special education, rehabilitation counseling, social work, or closely related field required
  • Minimum two years of experience working with persons with brain injuries
  • Prior experience developing and marketing community-based services and knowledge of case management practices highly preferred
  • Ability to acquire a CBIS (certified brain injury specialist) or QBISP (qualified brain injury support provider) certification within the first year of employment
  • General understanding of a broad range of disabilities, including brain injuries, substance abuse, mental health, cognitive disabilities, and physical or sensory disabilities
  • Excellent oral and written communication skills
  • Capable of establishing and maintaining effective working relationships with staff, families, referral sources, and members of the community
  • Ability to organize, manage, and prioritize multiple tasks and quickly adapt to changes in job routines
  • Effective use of technology and information systems including, but not limited to, Microsoft 365

 

PHYSICAL DEMANDS & WORK ENVIRONMENT:  

Physical demands: Performing the duties of this job requires near visual acuity, the ability to hear, understand, and distinguish speech and/or other sounds, and gross manipulation. Additionally, the employee:

  • Must be free of tuberculosis in a communicable form
  • Must be able to lift 20 pounds
  • Must be able to sit or stand for prolonged periods (up to two hours)
  • Must be able to work a flexible schedule including evenings and weekends
  • Must be able to provide direct support services for individuals with physical and/or intellectual disabilities which may involve crouching, stooping, or reaching/below shoulder level and overhead 

Work environment: While performing the duties of this job, the employee may be in an office or indoor setting or externally in community settings. Indoor environmental conditions will vary according to the weather conditions prevalent at the time and exposure to inclement weather and extreme temperature variations can be expected when in community settings. The noise level in the work environment will vary. While working with individuals with mental/physical disabilities, exposure to physical risks from behaviorally challenged individuals and contact exposure to human pathogens and/or biohazards is possible
