Classification Officer

This position will provide direct case management to juvenile clients for the Youth Diagnostic & Development Center and provide proper classification of clients within Juvenile Justice Facilities or community.

This position will provide quality case management of all clients from arrival in assigned unit, Supervised Release, transfer or discharge. Will establish and facilitate multi-disciplinary team meetings, ensuring the client needs are identified and addressed through Structured Decision Making Tool and facility Plan of Care according to policy and procedures. Will be the liaison between the community providers to ascertain that the client(s) have solid aftercare to include placement, treatment, education, vocational employment, and supervision components upon release and/or discharge. Ensure that all documentation in regards to Supervised Release Reports, FACTS Progress Reports, transfer requests, FACTS weekly narrative note, schedules and plans of care are completed and relevant to the clients case.
