Home Health - Full Time - RN Case Manager

Location: Fortworth, TX, 76102

Our home health clinicians work with physicians to provide in-home rehabilitation to our patients. Together, we will help our patients achieve the outlined goals and milestones on their patient care plan.

Job Summary

The Home Health RN Case Manager is responsible for providing skilled nursing care to patients in their homes. The RN Case Manager will assess, plan, implement, and evaluate patient care plans and collaborate with other healthcare professionals to ensure high-quality patient care.

Responsibilities

  • Conduct initial and ongoing assessments of patient needs and develop individualized care plans
  • Provide skilled nursing care, including wound care, medication management, and patient education
  • Collaborate with physicians, therapists, and other healthcare professionals to coordinate patient care
  • Monitor and document patient progress and adjust care plans as needed
  • Ensure compliance with all state and federal regulations and agency policies and procedures
  • Provide education and support to patients and their families
  • Participate in on-call rotation as needed

Qualifications

  • Current RN license in the state of Texas
  • Minimum of 2 years of experience in home health or a related field
  • Excellent communication and interpersonal skills
  • Strong critical thinking and problem-solving skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation

Physical Demands

The Home Health RN Case Manager must be able to lift and move up to 50 pounds and be able to stand, walk, and sit for extended periods of time.

Work Environment

The Home Health RN Case Manager will work in patients' homes and may be exposed to infectious diseases and hazardous materials. The RN Case Manager must follow all safety protocols and wear appropriate personal protective equipment.
