Job Title: Community First Choice (CFC) Universal Assessor Care Manager

Reports To: PrCA/CFC Supervisor FLSA Status: Exempt
SUMMARY
Upon referral from DSS or other contracted entity, complete all activities needed to assist a consumer in completing the Universal Assessment. Educate the Consumer on which services and supports may be available based on the findings of the Universal Assessment. The CFC Assessor will work with self-directed consumers and, to that end, will educate consumers on this model of community living.
ESSENTIAL RESPONSIBILITIES include the following. Other duties may be assigned.
- Working with the Consumer and, as appropriate, their representative, complete the Universal Assessment.
- Coordinate home visits to consumers honoring all timelines for work completion and submission.
- With the Consumer’s consent, participate in the activities aimed at the successful completion of a Universal Assessment including but not limited to conferring with medical staff, reviewing medical or psycho-social records/reports, conferring with other community providers as well as the consumer’s support system.
- Develop client plan of services as indicated/requested.
- Educate clients about community supports available and the self-directed model of community living.
- Submit required documentation to the DSS and or the contracted entity, honoring all mandated timelines.
- Maintain a system for tracking assignments.
- Maintain complete documentation of consumer related activities according to documentation guidelines.
- Participate in all educational programs designed to provide education on the Universal Assessment and related programs and assist the Coordinator in sharing this information with other assigned AASCC staff.
- Represent AASCC at meetings as requested.
- Assist in the submission of reports.
- Assist the Coordinator in the development of systems for new programs.
- Assist the Coordinator in mentoring new staff.
- Participate in various program committees as assigned.
NON-ESSENTIAL RESPONSIBILITIES
- Filing, typing and copying.
- Must be computer literate, familiar with Word programs.
SUPERVISORY RESPONSIBILITIES
Responsible for own work and assisting in mentoring newer staff to Community First Choice.
EDUCATION and/or EXPERIENCE
- Registered Nurse licensed in Connecticut or a graduate of a four year college or university with a degree in social work, social services or psychology.
- One to two year of experience required dependent upon degree.
LANGUAGE SKILLS
Ability to read and interpret documents such as business correspondence and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of consumers or employees of organization. Ability to write/document clearly, concisely and legibly.
MATHEMATICAL SKILLS
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent.
REASONING ABILITY
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
SCOPE OF RESPONSIBILITY
Employees functions independently within broad scope of established departmental policies and goals, generally refers specific problems to coordinator only where clarification of operating policies and procedures may be required.
INTERPERSONAL SKILLS
- Contacts involve the ability to influence actions of others where considerable perception is required to analyze individual circumstances and determine most appropriate course of action.
- Must be able to communicate routine information in a clear and courteous manner.

Valid State of Connecticut Driver’s License
PHYSICAL DEMANDS and WORK ENVIRONMENT
While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee is occasionally required to stand; walk; use hands to finger, handle, or feel; reach with hands and arms; and climb or balance, and must occasionally lift and/or move up to 10 pounds. Specific vision abilities required by this job include close vision, and distance vision. Approximately 30% of work time is spent driving to client residences to conduct required home visits. The employee is occasionally exposed to outside weather conditions. The noise level in the work environment is usually moderate.
