Service Coordinator

Job Title: Service Coordinator

Location: Crown Heights - with consistent travel for patient visits throughout the 5 boroughs (hybrid option available after 4-6 weeks of in-person training) 

Hours: 9am-5pm, Mon-Fri
Compensation: $45,000-$50,000 annually

Position Summary

The Service Coordinator manages a caseload of 25 patients, ensuring they receive the resources and support necessary to remain in their homes. This role combines task-based responsibilities, such as coordinating supplies and transportation, with problem-solving to address patient concerns and ensure program retention.

Key Responsibilities

  • Maintain regular communication with patients to address needs.
  • Prevent discharges by troubleshooting and resolving issues.
  • Coordinate quarterly home visits and complete timely documentation.
  • Use internal systems like Bolt, HHAX, and Smartsheets to manage patient needs.
  • Facilitate patient access to resources such as supplies and transportation.

Requirements

  • BA Required
  • 2+ Years in customer service type role.
  • Must be in NY and able to travel throughout the 5 boroughs (visiting 25 patients once every 3 months).
  • Must be available for on-site training in Crown Heights 9am-5pm for 4-6 weeks.
  • Strong communication and problem-solving skills.
  • Ability to use or quickly learn CRM systems.
  • Empathy, adaptability, and a customer-first mindset.
