Assistant Medical Director- Advanced Practice Clinician

The Community Health Centers of Lane County (CHCLC) is seeking an Associate Medical Director APC (AMD) to be part of an integrated team that provides a combination of leadership and primary care within the Patient Centered Medical Home model. This mission driven individual will work as part of the leadership team and provide intermittent clinical care at one of the CHCLC eight clinics. They will provide professional, comprehensive primary care or pediatric services to a diverse population of patients in a community health center setting using a team-based approach approximately 10% of the time. As a primary care provider, the AMD treats a wide variety of health problems, provides counseling and referrals for patients when appropriate, and performs related duties as assigned. As a leader, the AMD collaborates with the executive leadership team to help lead the creation, implementation, and ongoing assessment of medical policies for our primary care teams. The Assistant Medical Director APC also will provide oversight to the Quality and Performance Improvement initiatives of the CHCLC including program management, project oversight, clinical guidance, and process improvement.

About the Position

The AMD will supervise clinical staff and assigned support staff, including conduct performance evaluations, ensure staff follow policies and procedures, and make hiring, termination, and disciplinary decisions. The AMD will also provide supervision to medical, administrative, and supervisory staff within a clinical or public health office and oversee the day-to-day work and clinical administration of a medical care team, including providing technical direction and problem solving, and provide direct patient care in accordance with training, experience, and licensure. This position works collaboratively throughout the organization to ensure the adoption of standards and policies for the functional aspects of clinical services, fostering excellent patient care. The Assistant Medical Director will be part of the leadership team that is responsible for individual clinicians, programs, or practice areas and report to the Medical Director.  

As a primary care provider, as needed, the AMD will examine individuals requesting care, diagnose their medical conditions, prescribe and carry out, or direct others in carrying out, appropriate treatment, or refer individuals for specialty consultation or treatment in conformance with approved clinical protocols and guidelines; record patient transactions as they occur in the patient’s record; and complete referrals and data collections. 

Other duties include the following:

  • Plan, prioritize, distribute, coordinate, and monitor work assignments for assigned staff, including assign resources, review work product, and provide feedback and guidance. Supervise the day-to-day clinical activities of assigned providers and staff.
  • Collaboration with the management team in oversight of clinical policies, quality initiatives, onboarding and competency assessment, and procedure development.
  • Provide regular progress reports to the management team on clinical and quality initiatives
  • Prioritize and coordinate the delivery of services and activities, assess outcomes, and recommend process improvements.
  • Recommend changes in all clinical areas, and/or in general areas. to improve patient flow, medical records, care quality, and appointment practices

About the Division

The Community Health Centers of Lane County (CHCLC) is a dynamic organization that has been providing comprehensive primary care and behavioral health services to the citizens of Lane County since 2004. We operate eight primary care clinics in which we provide a broad array of services to adults and children that are organized around care teams who work collaboratively to support our patients. Our integrated care teams include primary care providers, nurses, pharmacists, medical assistants, patient care coordinators, community health workers, and behavioral health specialists. This model of primary care prioritizes the patient, who has increased access to other services beyond general medical care at our clinics. Our care teams also help patients reach their optimal health status by addressing social determinants of health barriers to care such as transportation, food insecurity, or housing. The CHCLC provide affordable primary care for the whole family – infants, children, adolescents, adults, and seniors – as well as alternative services such as preventative dental services. At the Community Health Centers of Lane County, we focus on integrated care that values the whole person. Our services encompass not only treatment and prevention of illness, but also health education and behavioral health services. 

We also provide preventative dental services to children in schools, Head Start, and WIC locations throughout Lane County. CHCLC provides services to approximately 30,000 individuals each year and is a primary source of care for uninsured and underinsured individuals. CHCLC is a Federally Qualified Health Center whose mission is to serve the community by offering excellent care and eliminating barriers to healthcare. QUALIFICATIONS:

Education and Experience:

  • Bachelor’s degree or equivalent from an accredited college or university with major course
    work in primary care program for Physician Associates, or completion of a Nurse Practitioner
    program specific to the expanded specialty role.
  • Four years of responsible experience in medical care, including one year of experience in a
    supervisory, management, or leadership role; or a combination sufficient to successfully
    perform the essential duties of the position.
  • Or an equivalent combination of experience and training that will demonstrate the required knowledge and abilities is qualifying.

Special Requirements:

  • Possession of a State of Oregon Physician Associate or Nurse Practitioner license.
  • Possession of a Drug Enforcement Agency number at the time of appointment.
