Patient Care Coordinator Internship

Coordination of Care:

Develop and manage patient care plans, ensuring patients receive necessary medical treatment and follow-up care.

Schedule appointments, manage patient records, and ensure efficient workflow within the healthcare facility. 

Patient Education:

Problem Solving:

Address patient inquiries, concerns, and issues promptly and effectively. 

Data Management:

Maintain accurate and confidential patient records, including medical history, test results, and treatment plans. 

Referrals and Authorizations:

Assist with making referrals to specialists and obtaining necessary authorizations for medical procedures.
