
Case Manager

Our Case Mangers provide mental health, substance use and outreach services to individuals who may have a mental health and/or substance use disorder. This role works with community partners and local stakeholders to provide comprehensive care. A variety of clinical training opportunities are available based on program and interest.
ESSENTIAL DUTIES & RESPONSIBILITIES
- Provide individual and group mental health and substance use disorder case management services. Refers, advocates, and links client and family for additional services as identified.
- Provide supportive and educational counseling services (individual, family, and group) to develop daily living and coping skills. Assist clients with developing natural support systems.
- Transport clients in company vehicles to necessary appointments as needed.
- Develop, implement, evaluate and update treatment plans with input from the client and family.
- Conduct clinical behavioral assessments.
- Act as liaison and communicate collaboratively with local law enforcement, and local stakeholders to maintain coordination of the treatment process.
KNOWLEDGE, SKILLS & ABILITIES
- Good organizational, time management, written and verbal communication skills.
- Ability to develop supportive relationships with persons with mental health and substance use disorders while maintaining appropriate therapeutic boundaries.
QUALIFICATIONS
- Bachelor's Degree required.
- Community-Based program case managers must have a valid FL driver's license.
PREFERRED QUALIFICATIONS
- Knowledge of community resources or ability to learn quickly.
- Experience working with individuals with mental health or substance use concerns.
- Enthusiasm about influencing positive change in client's lives.

PAY RANGE
- $16.66 - $20.00 per Hour
