Neighborhood Health Care Housing Navigator

JOB DESCRIPTION.

Neighborhood Health Care Housing Navigators are community-based care managers providing intensive care management support to community members through intensive coordination of housing and health-related services via Enhanced Care Management (ECM). The Neighborhood Health Care Housing Navigator provides outreach and engagement, assessment and care management, enhanced coordinated care, health promotion, transitional care, family support, address housing needs, and coordination of and referral to community and social support services. This role interacts directly with the member and/or family member(s), guardian, authorized representative, caregiver, and/or authorized support person(s), as appropriate, and coordinates all covered physical, behavioral, developmental, long-term services and supports, specialty mental health services, housing, community supports, housing tenancy and stability, and other services that address social determinants of health needs, regardless of setting.

 

JOB COMPLEXITY.

  • Serve as a community-based advocate and resource for health plan members, using knowledge of the community and resources available to engage and assist clients in managing needs.
  • Engage clients with motivational interviewing and trauma-informed care practices to conduct initial intakes, confirm program eligibility, provide ongoing outreach, and schedule ongoing appointments.
  • Contact eligible members to assist in development, implementation, evaluation, and revision of Assessments and individualized Care Plans and objectives 
  • Empower clients by helping them navigate and maximize their health plan benefits. Assistance may include: scheduling appointments with providers; arranging transportation for healthcare visits; getting prescriptions filled; and following up with clients on missed appointments.
  • Identify continued service gaps and needs as defined by the community and communicate those to the team


 

MINIMUM COMPETENCIES

  • Master’s Degree in Sociology, Social Work, Child Development or a related field preferred OR a combination of education and at least 5 years social services experience working with youth and families
  • Experience providing case management/community health work services to underserved or special needs populations with varied and complex health, behavioral health, economic and educational circumstances
  • Bilingual Required (Spanish preferred)
  • Personal experience living or working in the community to be served, including awareness of available resources, community conditions, barriers, and assets.
  • Experience working with diverse populations and multiple systems (ie. housing, employment, etc.)
  • Must have reliable transportation

     

COMPENSATION

Starting Rate: $27.17 per hour (all new hires begin at starting salary regardless of experience)
