Minimum Qualifications of Education and Experience
A bachelor's degree from an accredited four-year college with a major in social work, psychology, sociology or a closely related field.
OR
A bachelor's degree from an accredited four-year college with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
Regular and reliable attendance is a necessary component of job/position. Individuals required to use County vehicles and equipment must have a valid driver's license and be free of any major traffic violations for the last three (3) years.
Desirable Qualifications:
Two years of social service-related experience.
Nature of Work
Expected starting salary is $33.45 to $44.66. Full salary range is $33.45 - $55.87.
Under supervision of a Program Manager or Supervisor, and within Federal, State and locally developed programs and policies, supporting individuals diagnosed with a disability. Identifies individual needs and works toward meeting these needs through county and community support services. Will be working within a multi-disciplinary team setting. Supports a diverse, respectful, and inclusive workplace. The services help a person live as independently as possible in community settings and promote optimal health, independence, safety and community integration.
Examples of Work
- Provides assessment and case management for individuals served through Waiver Programs.
- Uses revised MN CHOICES assessment tool to determine the functional needs and abilities of the individual to determine eligibility for services.
- Works with a variety of funding programs/sources (County, State, and/or Federal) to meet the individual's needs.
- Develops and writes community support plans.
- Advises individuals, and their families of possible resources, rights and responsibilities under programs or services.
- Works with individuals, team members, and providers who support the individual.
- Refers individuals to other programs or agencies when appropriate.
- Utilizes State and Local computer programs to document work for the county and the individual.
- Maintains case records in accordance with State, Federal, and local guidelines.
- Performs all other duties as assigned.
Knowledge, Skills and Abilities Required
- Knowledge of a variety of program and services for individuals with disabilities or related conditions, including knowledge of physical health, mental health, and chemical health needs.
- Knowledge of the dynamics of individual and group behavior.
- Knowledge of the principles, methods and techniques of social work
