Hours: We are hiring for a part-time position. Two Evening Weekday Shifts/Week, 4PM- 11:30PM EST; One Weekend Daytime Shift, 11AM - 5:30PM EST.

Week #1
Tuesday: 4pm-11:30pm
Thursday: 4pm-11:30pm
Saturday: 11am-5:30pm
Week #2:
Wednesday: 4pm-11:30pm
Thursday: 4pm-11:30pm
Sunday: 11am-5:30pm

As a Compassionate Connector you will be responsible for serving as a liaison/connector between employees of a large retail client, and the community. You will help to bridge conversations with employees and remove barriers that prevent them from accessing health and social services. You will conduct telephonic and/or face-to-face outreach to employees for appointment scheduling, social determinant of health needs assessment, and care gap closure.
How you will make an impact:
- Meets employees in assigned client locations to help identify social determinants of health impacting patient’s health and general well-being.
- Collaborates with social worker and/or care manager to develop action plan.
- Develops trusting relationship with employees by providing support and advocacy to help achieve health goals.
- Builds strong relationships with employees and employer management, through walking the on-site location and interacting with team members.
- Assists employees in accessing health-related services and community resources, such as assistance with enrollment forms.
- Facilitates communication between all parties (employees and community-based organizations) as needed.
- Documents interactions in electronic medical records and completes all other required documentation in a timely manner.
- Attends meetings and completes trainings as required.
Minimum Requirements:
- Requires a high school diploma and a minimum of 3 years related experience; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities and Experiences:

- BA/BS degree in human services or public health preferred
- Experience working in a Community-based or Social Work settings, or experience in Case Management Care Coordination and/or any combination of education and experience which provide an equivalent background.
- Adept at all MS Office products, including Excel and PowerPoint.
- Experience with EMR software platforms
- Bilingual candidates preferred
