Aging Outreach Services is a full-service elder care firm that was established in Moore County, NC in 1999. In our 25 years of service we have helped many students create a path to a career as a care manager, serving the older adult population. Our care management team is made up of social workers, gerontologists and other social service professionals. We are currently looking to expand our team, and a new graduate looking to establish a rewarding career is just what we are looking for!
What is a Geriatric Care Manager (GCM)?
A geriatric care manager is an individual that specializes in coordinating and maintaining the complex needs of older adults and their families. This includes managing medical and social needs through assessments, creating care plans, and connecting them with the appropriate resources in their community. These are individuals who fill the gaps in what a family might not be able to provide for their loved one.
What is a Care Management Associate (CMA)?
A care management associate is an individual who holds a 4-year degree in a human services study. They are willing to learn the role of a GCM and get to know the community they are working in. At Aging Outreach Services, individuals would spend a year in the role as a CMA learning the company and how we operate, as well as building connections in the community so that they can build their own caseload. The benefits of this position are that there is no pressure to bill a certain number of hours right away, one has the time to learn and ask questions, and can get to know a professional network that they may not be familiar with.
Care Management Associate to Geriatric Care Manager Track
Aging Outreach Services has a unique opportunity available for an individual to work towards becoming a Geriatric Care Manager. The track that has been developed and proven to be successful as an individual starting as a Care Management Associate for one year, then moving into a role as a full-time Geriatric Care Manager. Aging Outreach Services is looking for someone who is finishing a four-year degree in a human service study (or related field) and is interested in working with the aging population in Moore County, North Carolina.
The most recent individual who went through the CMA to GCM track at Aging Outreach Services was able to spend their first year becoming acquainted with the professional community in addition to AOS clients, familiarizing themselves with how AOS operates within the community, and acquiring the necessary skills to manage documents and communication systems.
A Care Management Associate to Geriatric Care Manager Success at AOS
AOS Care Manager Ashley Seace holds a Bachelor of Arts degree in Psychology. When moving into the role of GCM, Ashley was able to successfully build a caseload and have referrals coming in on a recurring basis. Since working at AOS, she has become a Certified Care Manager through the National Academy of Certified Care Managers.
