Service Coordinator/Case Manager - Washington/Greene County

Due to company growth, we are looking for candidates to fill caseloads in the following Counties: Allegheny, Armstrong, Beaver, Dauphin,  Greene, Indiana, Luzerne, and Washington.

WHO IS Alleghenies United Cerebral Palsy (AUCP)- AUCP is a non-profit organization founded in 1955. It started as a grassroots effort by a group of concerned parents who wanted to find support for family members aged 18 – 59 with physical and intellectual disabilities.  Starting in a church basement, we have evolved into an organization serving over 4,000 lives 18 and older, with staff throughout PA. Our mission is to assist individuals with disabilities in meeting their basic needs, enhancing their quality of life, and promoting their independence while also allowing them to be active participants in the decision-making and direction of their services.   

Job Responsibilities:

  • SC meets with teams of clients and develops a plan that reflects the individual's priorities and how the team will assist with meeting those goals.
  • SC updates the plan as needed to reflect what is currently happening in the individual's life and update services.
  • SC coordinates and monitors services based on program regulations with paid providers and community resources to support the individual's needs, ensure they are meeting them, utilize them according to authorization, and ensure that staff are knowledgeable and following the service plan.
  • SC communicates with teams as needed regarding updates, concerns, and issues and ensures they are addressed promptly and appropriately.
  • Follows timelines and guidelines determined by program regulations and Alleghenies United Cerebral Palsy, including documenting and keeping up with billing requirements.

Requirements:

  • Service Coordinators must possess a reliable motor vehicle and a valid PA motor vehicle license and be able to travel to clients.
  • Educational Requirements: A Bachelor’s degree (B.A.) from a four-year college or university in a Social or Human Services or related field is required (i.e., social work, psychology, education, etc.).
  • At least three (3) years of professional experience in case management and/or Three (3) years of experience in working with individuals with Autism.
  • Must pass an MA Fraud Check,  PA State Police Background Check (e-PATCH), Child Abuse clearance, FBI check, Social Security Verification, and a DMV check.

Job Type: Full-time

Starting Hourly Rate: 19/hr, with the ability to get a raise to 20/hr after a probationary period and performance review

Work Location: Hybrid/Remote Position with the ability to complete visits in the community in PA.  Must live in PA or Willing to travel to PA to complete home visits
