Pines to Palms Geriatric Care Management provides services to aging and disabled adults in Florida (Pinellas, Pasco, Hillsborough, Manatee and Sarasota counties) in addition to Michigan (Macomb and Oakland counties). Our Geriatric Care Managers manage our client’s healthcare affairs. We offer referrals to community resources, coordinate and oversee all services. Pines to Palms is the liaison between seniors, their family members and their care team, as we provide our general oversight and communicate changes as needed to provide peace of mind to all those involved in our client’s lives.

The Professional Care Manager provides comprehensive care management services to a variety of clients. A typical caseload is 1-10 clients which may include the elderly, the developmentally disabled, the mentally ill, or individuals with problems of chronic physical ailments and/or disabilities. Clients may live at home or in any type of community or institutional setting.

Job Type: PRN, flexible self-determined schedule, regular business hours and/or after hours, weekends available. Combination of in-person home visits and community visits, and work-from home/remote work.

Essential Duties & Responsibilities:

· Identify and assess our patients with complex needs

· Provide an in-home assessment to identify needs and suggest the best route of care client by creating a personalized care plan primarily in person, however the following may be necessary: telehealth/video/virtual; phone calls; emails

· Provide one-on-one tailored care management to create a care team and services including accompanying clients to medical appointments

· Suggest and coordinate referrals including elder law professionals, caregiving companies, financial resources and more, depending on the assessment of client personal needs

· Suggestions for alternate living arrangements and assisting with a transition

· Communication for the healthcare needs to family members, caregivers and medical professionals involved in our client’s care

· Researching and providing community resources

· Document and updates activity in EHR to ensure compliance with company protocols

· May perform on-call services to ensure 24-hour availability for concerns after business hours.

Knowledge and Skill Requirements:

· Bachelors or Master's (preferred) Degree, or in pursuit of, in social work, gerontology, nursing, psychology, physical therapy, occupational therapy, health and human services, or a related field of human services delivery. Fully licensed Social Workers encouraged to apply.

· Helpful but not required is knowledge and experience in working with Guardianship, Conservatorship, Trusts, and related fiduciary issues.

· Current State driver's license and a registered, insured vehicle with the ability to move between locations

· Experience in Chronic Care Management a plus

· Case Management or related Certification a plus

Assistance with CMC certification and ALCA (Aging Life Care Association) application and LMSW license given.
