Site Coordinator
Job Purpose/Summary
Responsible for leading or co-leading a school site team in the implementation of the CIS Model at a comprehensive CIS of Nevada site. Serve as a liaison between the CIS of Nevada comprehensive site school leadership team and the CIS of Nevada leadership team. Responsible for developing, planning, implementing, and supervising the tasks associated with the annual School Support Plan to include Tier I, Tier II and Tier III services. Work closely with school staff, parents/families, community agencies and students to assure coordination of services for the benefit of the students and their families. Perform any and all reasonable and work-related duties assigned by CIS of Nevada leadership. Prior to receiving a school assignment, a coordinator will shadow and receive training until site placement. The unassigned coordinator is responsible for knowing how to perform the duties and responsibilities of the position and is responsible for providing support at school sites as needed for assigned coordinator absences, event assistance, and ongoing support of the Site Coordinator duties and responsibilities.
Duties and Responsibilities
Campus Planning & School Relations
- Assess the current campus needs with administration, teachers, school staff, parents/guardians and students, and coordinate community resources where possible to provide the needed services.
- Evaluate campus-specific and student-specific needs and barriers and coordinate resources to meet the needs and assist with removing barriers.
- Complete School Needs Assessment and School Support Plan within established timelines.
- Plan and implement special events and guest speakers for students with pre-approved programs as part of the CIS School Support Plan.
- Plan parental involvement activities and engage parents as part of the CIS School Support Plan.
- Assist CIS of Nevada leadership in collaborating with community agencies and fostering new community resources.
- Collaborate with school staff to provide or broker appropriate services to address attendance, behavioral, course performance, and physical or social services for students in need and ensure compliance with school district policies and procedures.
- Promote cooperation between school staff, administration, and the CIS of Nevada team members.
- Collaborate with school staff to integrate the CIS of Nevada Tier I, Tier II and Tier III services, and keep informed about pertinent activities occurring at the school and in the community.
- Network with school staff to create and maintain a positive image of CIS of Nevada.
- Provide desired feedback to the school and agency personnel.
- Communicate regularly with the school administration and staff.
- Maintain accurate record of in-kind donation process and documentation.
- Participate in school assigned duties and responsibilities.
Case Management
- Recruit and enroll students with consent of parent/guardian through a signed Parent/Guardian Consent Form and within the other CIS National Accreditation Guidelines.
- Coordinate and complete an individual Student Support Plan for all Tier Integrated Supports for case managed students.
- Supervise attendance, behavior, course performance and/or social emotional goals for each student.
- Complete all forms and documentation according to program guidelines and timelines.
- Provide and document all services per program requirements. Provide and document all Tier II/III and check-in services per program frequency requirements.
- Keep all documentation current, which is within 30 calendar days of the service delivery.
- Access and coordinate community health and human service agencies for services delivered for the benefit of CIS of Nevada students and their families.
- Enroll students for Tier II and Tier III integrated student support services sufficient to meet required program caseload requirements as set by CIS National Accreditation Standards and CIS of Nevada guidelines.
- Perform mandated reporting for Case Management and Tier I supports.
- Facilitate annual transition of case managed students promoted to feeder schools, facilitating a soft hand off to the feeder school Site Coordinator through introductions and case notes.
Data, Documentation & Reporting
- Complete case management documentation in the required format, within established timelines.
- Complete monthly and quarterly reports in the required format, within required timelines.
- Complete enrollment of all new and returning students into the data entry system as required. Meet caseload requirement as established by affiliate.
- Enter service data for all CIS of Nevada students in the data entry system, within 30 days of the service delivery. Review entries to ensure accuracy and completeness.
- Ensure all the following are entered on time and with accuracy: school documents (School Needs Assessment, School Support Plan, Signed School Agreement), caseload requirements, Tier I/II/III support entries, basic needs entries, site coordination entries, quality case notes, quarterly progress monitoring and progress reports, report procedures, case file requirements into CIS of Nevada data system.
- Responsible for retrieving and reviewing CISDM reports to monitor errors and provide responses to specific data questions.
- Report to CIS of Nevada leadership issues affecting school relationships and emergency information.
- Complete required correspondence and communication, including telephone and e-mail correspondence.
- Respond to and complete special reports as required by CIS of Nevada leadership.
- Enter all progress monitoring data quarterly and end of year data within established timelines.
- Share quarterly progress reports with school leadership, school support team, and affiliate leadership within established timelines.
- Immediately report all problems with the data entry system to the Data Department.
Meetings, Training, and Professional Development
- Attend all CIS of Nevada Site Coordinator meetings.
- Participate in CIS of Nevada programmatic, data and professional development training.
- Complete Site Coordinator Learning Pathway certification through CIS University within required timeline and required CIS Nevada State Standards trainings with verification annually.
- Travel & report to various sites carrying out job duties and responsibilities assigned by CISNV Leadership Team.
- Additional duties as assigned.
- Complete school district training as required.
Qualifications and Competencies
Qualifications
- Associate degree or equivalent experience in the health and human services (or a closely related) field.
- Experience working with at-risk youth and case management for at least one year preferred.
- Excellent interpersonal, written and verbal communications skills, along with computer and database skills.
- Must be able to pass pre-employment criminal background check, fingerprint screen and drug testing.
Work Environment & Physical Demands
- Work environment is indoors and climate controlled. Frequently exposed to outdoor weather conditions.
- Travel required to and from school sites.
- Moderate noise levels as found in a business office and classroom setting.
- Use of own automobile.
