Social Determinants of Health Internship - Sixteenth Street Health Center (SEJ Program)

Social Services: SDoH Intern Position

Role: Social Determinants of Health Intern


To better address the overall health of our patients and clients, Sixteenth Street is working to integrate Social Determinants of Health into the clinical setting by screening adult patients for current needs, identifying helpful resources, and making connections to additional services. The intern will serve as a Patient Screener and contribute meaningfully to additional projects relating to social determinants. As a patient screener, the intern will gain experience in a busy, multi-cultural community health setting working with a medically underserved population. They will acquire valuable skills in customer service and patient interaction, witness first-hand the patient’s barriers to accessing care and maintaining wellness, and learn about health care coordination and availability of resources (or the lack of) in the Milwaukee area. The intern will have the opportunity to practice Spanish language skills.


The intern will:
• Work in communication with a primary care provider to connect with patients
• Screen patients using the Social Determinants of Health screening form
• Provide basic resources and referrals as appropriate to address identified needs
• Complete a shift summary after each shift


The intern will also create an individualized project plan determined by the site supervisor and the intern. Opportunities for projects include: data analysis and reporting, resource development, patient outcome follow-ups, patient intake coordination, ongoing training related to social determinants, shadowing, and informational interviews. The intern will work collaboratively alongside staff members and other interns to determine project goals and desired outcomes for these projects. The individualized project plan will be created by the intern and the Intern Coordinator at the beginning of the internship.


Requirements:
• Interns must speak Spanish at a conversational level or higher
• Intern will complete mandatory 1-hour Human Resources/Intern Orientation (including HIPAA/Confidentiality) and a 2-hour Social Services training on relevant practices
• A PPD (TB) test is required at no cost. In addition, intern must provide proof of MMR, TDap, and varicella vaccinations
• Intern must consent to a background check, performed by Sixteenth Street

Compensation:
This internship is part of the Lawrence University Social & Environmental Justice Cohort, which provides an opportunity for Lawrentians to explore social and environmental justice impact careers, gain professional experience, and develop as leaders. Students complete a full-time summer 2025 internship with an identified social and/or environmental justice impact organization in Milwaukee or Appleton, while participating in ongoing reflection with fellow students, Professor Jason Brozek, the Career Center, community leaders and alumni.  Students may apply for as many positions as they are qualified for and interested in. Each student in the SEJ program will receive a $5k stipend to help cover housing and transportation costs and living expenses.
