- 36.25-hour work week, full-time role. Hourly position paid bi-weekly.

- Starting salary range, determined by formula accounting for years of experience: $45,240.00 (base rate) to $56,550.00 (10 years of experience performing this same job or job with primarily the same duties.)

Transition Navigator
As a Transition Navigator, your day begins by reviewing updates on the individuals you support, ensuring their progress aligns with person-centered goals. Managing a caseload of 75-100 students, you will coordinate team meetings with individuals, their families, and service providers to refine their career exploration plans. Using an agency-provided laptop and iPhone, you’ll connect virtually or meet in person based on the individual’s preference. Your day might include visiting a volunteer site to observe a student gaining hands-on experience, followed by documenting their progress and updating their service plan. You’ll also work closely with schools and employers to arrange internships, provide guidance on employment opportunities, and maintain regular communication with families to address questions or concerns. Every day brings an opportunity to make a lasting impact on someone’s journey to independence and employment.
Job Duties (Summary):
- Assist individuals in identifying their interests, strengths, and career goals; connect them to pre-employment and employment services using a person-centered approach.
- Serve as the primary point of contact for assigned individuals and families, collaborating with interdisciplinary teams to ensure individualized outcomes.
- Guide individuals and families in selecting and obtaining services, coordinating funding, and monitoring waiver and employment supports.
- Document service provision and maintain progress notes for each individual on the caseload, ensuring compliance with service requirements.
- Support students aged 14–21 in activities like summer programs, job shadowing, and internships, providing information on related services and providers.
- Provide ongoing transition planning and connect individuals with employers for volunteer and paid employment opportunities.
- Stay informed on best practices in transition navigation and participate in professional development.
- Performs additional duties as required or assigned.
Requirements:
- Bachelor's degree required, a degree in behavior analysis, vocational rehabilitation, psychology, special education, or related field preferred.
- One (1) year of full-time paid work experience in programs or services for individuals with developmental disabilities.
- Eligible for and able to obtain within one year of hire Ohio Department of Developmental Disabilities’ Service & Support Administration Certification.
- Must be able to pass criminal background checks and various abuser registry checks
