Case Manager / Career Counselor
The Case Manager takes an active role in assessing and assisting residents in the program to meet their Individual Treatment Plan goals in the areas of educational/vocational planning and employment goals. The Case Manager works as part of a team that develops a milieu where youth take responsibility for their behavior and decisions.
Responsibilities
- Develop a network of Vocational training and job placement resources
- Assess residents for basic skills in education and vocation and coordinate, and monitor the educational planning for all clients.
- Supervise and implement tutorial services and mentoring for residents
- Conduct/coordinate employment and education workshops as needed.
- Provide aftercare services for former residents.
- Assist residents in obtaining necessary documentation and health insurance.
- Perform Step Up to Success (SU2S) assessments, input all data/information and assign SU2S tasks. In addition, monitor client's progress in SU2S and attend meetings.
- Enter in case notes in the HMIS system.
- Conduct intakes for residents into the program when needed.
- Participate in monthly case reviews for each resident.
- Assess the youth for services and make all necessary referrals in a timely manner.
Qualifications
- Bachelor's Degree in Human Service related field required, Master's degree preferred.
- Minimum of 2 years of experience with adolescence in a residential environment
- Excellent verbal and written communication skills.
Basic computer skills with MS Office - Valid and clean driver's license
