
PACT Case Manager
MAJOR FUNCTIONS:
Provides NYCHA housing residents accessto services to allow them to stabilize their lives, sustaintheir lifestyle needs, and help them thrive to develop and grow themselves personally, financially, socially, and intellectually.
DESCRIPTION OF DUTIES AND RESPONSIBILITIES
- Helps connect residents to service, educational, financial, employment, housing, medical, mental health, and social opportunities.
- Creates an environment conducive to resident participation.
- Conducts outreach to residents to facilitate involvement.
- Maintains records of client services and activities.
- Maintains progress notes.
- In conjunction with other staff, establishes and maintains relationships with referral sources.
- Maintain a caseload of 20-25 clients plus participate in group meetings and service sessions.
- Assist other counselors with connections to linkages and referral sources.
- Report on activityto Program Director.
- Workin tandem with other staff to maintainand develop programactivities and new
- services.
