Case Manager - Brain Injury Services
Case Manager- Enjoy a rewarding and challenging position with a nonprofit organization that has been helping educate, train and provide employment opportunities for individuals with disabilities since 1955. We are currently seeking applicants for a full-time Brain Injury Case Manager Monday - Friday 8:00 AM -4:30 PM with occasional evenings and weekends.
PRIMARY FUNCTION
The primary responsibilities of this position include the assessment, coordination, and provision of services to adults living with brain injuries and their families. This position will also be responsible for the development and marketing of case management services within the established service area. These services are provided using a model of community-based case management.
WORK ENVIRONMENT
Work is performed primarily in a typical interior work environment. Work may involve transporting individuals to their homes, community activities, and other agencies. Universal precautions are needed to prevent exposure while performing personal care duties as needed.
ESSENTIAL DUTIES & RESPONSIBILITIES
1. Establish a solid therapeutic relationship with clients and their families. Communicate effectively with clients, their families, other support systems, and professionals. Maintains personal contact with the client and/or family at least monthly unless otherwise requested by the client and/or their family. Documents this in the Progress Notes.
2. Complete a thorough client assessment by:
- Assessing the client’s strengths and weaknesses, family and community support systems, existing resources presently utilized by the client, and gaps in services requiring intervention within 45 days of opening a case.
- Assessing assistive technology needs and seeking appropriate resources to meet needs.
- Obtaining appropriate releases, understanding the laws regarding confidentiality, and upholding these laws.
3. Completes all paperwork promptly and maintains documentation by agency and CARF standards.
4. Supports consumers in developing their own goals and plans based on the needs assessment, completes a Case Management Plan, and provides a written copy to the client.
5. Identify, coordinate, and monitor goal directed community services to meet the client’s goals and objectives as outlined in their plans.
6. Create and facilitate support teams to include referrals, scheduling of meetings, and delegation of duties.
7. Effectively negotiate for services for clients in their communities professionally.
8. Maintain a caseload of 25-30 active clients and 5-10 clients in a follow-along status; and provide education to clients, their families, and the community regarding brain injury and community resources.
9. Demonstrate a commitment to person-centered services and the opportunity for client choice in the selection of service providers as documented in the case management plan and progress notes.
10. Spend a minimum of 50% of their time in direct contact with clients and their families providing supportive counseling, coordination of services, resources determination, monitoring of services, and team meetings.
11. Monitor services by outside providers and advocates on behalf of their clients. Case managers advocate on behalf of their clients professionally to ensure services are provided in a way that meets the client's individual needs. This is documented clearly in the 90-day progress review.
12. Performs other duties as assigned by supervisor or designee.
QUALIFICATIONS
A minimum of a Bachelor’s Degree in special education, rehabilitation counseling, social work or a closely related field is required, plus two years’ experience working with persons with disabilities, preferably persons with brain injuries. Preference will be given to applicants with prior experience developing and marketing of community-based services and knowledge of case management practices. This position also requires certification as a Certified Brain Injury Specialist (CBIS). If the case manager is not CBIS certified at the time of hire, they will be required to obtain the certification through self-study and a passing examination within the first 18 months of employment. First Aid/CPR certified. Must have a valid driver’s license, access to a reliable car, and a positive driving record.
Physical Requirements:
Free of tuberculosis in a communicable form. Able to lift 30lbs and bend through a full range of motion, stoop, push, pull, climb stairs, and stand for an extended period (up to two hours). Able to communicate orally and in writing. Vision and dexterity sufficient to use a computer.
