Social Worker/Elder Care Advocate

Entry Level Full Time Elder Care Advocate (possible part time to full time or part time)

Seeking a social worker to join our Care Coordination team based in our Smithtown office:

• On site primarily in Smithtown office or in field (*not remote position)
• Support our care coordination team with in-home and virtual assessments of clients for the purpose of determining Community Medicaid home care services
• Work closely with senior social worker, attorneys, and paralegals to develop care plans for each client
• Assist clients and their families with enrollment calls to MLTC and referrals to waiver programs
• Educate and support clients and their families regarding local resources to help navigate aging in our local communities
• Work closely with various types of agencies to coordinate services for our clients
• Develop relationships with home health care agencies to assist families with staffing Medicaid approved home health aide hours  
 

EXPERIENCE (preferred but not required):
• Short-term and long-term care planning
• Knowledge of ALF, ALP, and other senior living options
• Experience navigating senior services throughout Long Island
• CDPAP/PCA services
