SIP 2025 - Community Pathways HUB Intern (Metro-MTC) at Tri-C Healthcare Solutions
The Cuyahoga Community College Community Pathways HUB is a community care coordination agency in partnership with the Better Health Pathways HUB, connecting at-risk populations to pathways for prenatal care, social services, medical referrals, education and employment resources, housing and more. Community Health Worker students complete their 130 clinical hours, required for certification through the Ohio Board of Nursing, by assessing the needs of students, their family and community members, locating resources, making referrals, and helping people to apply and navigate the systems providing the resources.
Note: In-person interviews will be conducted on Feb. 28, 2025, at West and Metro. These interview fairs are your only opportunity to meet and interview with hiring supervisors.
The Community Pathways HUB Intern would support the Community Pathways HUB department by assessing the needs of students, their family and community members, locating resources, making referrals, and helping people to apply and navigate the systems providing the resources. They will conduct outreach to increase awareness of the services of the HUB and provide a way to find people who need to navigate the Tri-C campuses.
Preferred Qualifications
- Proficient in MS Outlook, Word, and Excel.
- Empathic listener with the ability to listen and record information simultaneously and a desire to advocate for others.
- Ability to identify creditable resources, work autonomously and on teams.
- Ability and willingness to learn SAP Concur for data entry, reporting, and record keeping.
- Greet clients, answer phones and maintain cleanliness of assigned and common spaces throughout the day.

Program Requirements
- Must be enrolled in current semester (Spring 2025)
- Must enroll in a minimum of one Summer 2025 course
- Must be returning to Tri-C for Fall 2025 Semester
- Must have a declared major on record
This position is part of the Cuyahoga Community College Summer Internship Program.
