What We’re Looking For:

Seeking individuals who can make a difference and to be part of grassroots solutions for community development in defined health communities. Trillium Health Resources is a growth oriented Managed Care Organization that desires to improve our capability to address non-medical drivers of health for members in the communities we serve. The Trillium Neighborhood Connections Health Specialist will take part in efforts to address housing related challenges, transportation, food insecurity, employment and education challenges, interpersonal safety and toxic stress, as well as community inclusion and isolation. 

On a typical day, you might:

  • Receive referrals related to members’ unmet health related resource needs. 
  • Link members with relevant resources.
  • Represent Trillium Health Resources by involvement in conferences and meetings. 
  •  
  • Utilize designated software platforms to make closed loop referrals. 
  • Documentation in prescribed platforms.
  • Develop, present, and teach a variety of workshops to members.

Qualifications: 

Education:

 High School or GED

Experience: 

A minimum of one (1) year experience in the Health and Human Services field, including experience working with any combination of the BH/SU/IDD population and/or experience with public speaking/training and/or teaching. OR 

Equivalent combination of education/experience. 

Preferred Experience:

Prefer lived experience, Community Development experience, and/or experience with community-based organizations and/or community partners centered around housing, food insecurity, transportation, education & employment, interpersonal violence/toxic stress and/or social integration and inclusion.

License/Certification: N/A

Must have a valid driver’s license

Must reside in North Carolina to be considered for remote status
