Behavioral Health Specialist

Senior Care Partners PACE is seeking a dedicated Behavioral Health Specialist to join our dynamic Interdisciplinary Team (IDT). Under the guidance of the Associate Director of Behavioral Health & Social Services, you will play a vital role in providing compassionate care to our PACE participants facing behavioral health challenges.

Key Responsibilities:

  • Comprehensive Evaluations: Conduct thorough initial evaluations and develop personalized care plans that address behavioral health and substance use disorders for our participants.
  • Team Collaboration: Actively participate in regular IDT meetings, contributing to clinical discussions and sharing your knowledge to enhance team performance.
  • Participant Care Plans: Collaborate with the IDT to create and implement care plans, set meaningful goals, and track participant progress.
  • Therapeutic Services: Provide individual and group psychotherapy, leveraging your behavioral health expertise for impactful case consultations.
  • Crisis Management: Facilitate referrals to our Behavioral Health Nurse Practitioner and coordinate care with external services while providing necessary crisis intervention.
  • Education and Training: Deliver specialized training to your team on behavioral health topics, substance use disorders, and effective behavior management strategies.
  • Documentation and Compliance: Maintain accurate records of interactions and participant progress in accordance with policies, and engage in quality improvement activities.

Qualifications:

  • Master’s degree in Social Work (MSW) or a Licensed Professional Counselor (LPC) license in the State of Michigan.
  • A minimum of 2 years of behavioral health experience, with at least 1 year working with frail, elderly, or long-term care populations.
  • Excellent communication and interpersonal skills, with the ability to work independently and collaboratively.
  • Strong organizational skills to manage a busy work environment with frequent interruptions.

Work Environment:

In this role, you will have a visible presence within our center, engaging with participants, caregivers, and external organizations. Flexibility is key, as you may need to work beyond standard hours, including weekends and holidays.

Physical Demands:

  • Ability to assist participants with daily living activities, including lifting up to 50 lbs and performing tasks that may require bending, squatting, and prolonged standing.
  • Must be prepared to assist in emergencies and drive in various weather conditions.

Minimum Working Requirements:

  • Must be at least 18 years of age.
  • Must be fully vaccinated or willing to be vaccinated against communicable diseases, including Influenza, MMR, Varicella, Tdap/Td, Covid-19, and Hepatitis B.
