Job Summary

The primary purpose of this position is to serve as the installation non-clinical case worker for the Exceptional Family Member Program (EFMP). Serves as a liaison to and works closely with health care professionals on provision of available services and resources at the command to ensure best possible care in accordance with community, national and Navy standards. Assists in identification of community resources.

 

Duties

The FFSC is the Navy's primary agency for the delivery of an array of human services programs intended to enhance the stability and self-sufficiency of individuals and families, resulting in increased operational readiness. All FFR direct services are provided by the installation Fleet and Family Support Centers, and they fall into three main categories: Core Family Readiness, Navy Gold Star, and Sexual Assault Prevention and Response (SAPR). The majority of programs falls under the Core Family Readiness category and include sub-categories of Career/Retention, Deployment/Readiness, and Crisis Response. Diverse methods for delivering services include providing information and referral, educational instruction or training and professional counseling. Primary duties assigned to this position are, but are not limited to: Program Management (duties performed 20% of the time include but not limited to):

  • Implements the Exceptional Family Member Program (EFMP) in assigned Area of Responsibility (AOR) and conducts ongoing assessment of its consistency and effectiveness.
  • Identifies program needs and challenges and presents recommendations and assistance to influence policy compliance issues. Stays abreast of Department of the Navy (DON), SECNAV, OPNAV, and CNIC policies and interprets/articulates their meaning, priorities and intent.
  • Utilizes focus groups, in person contacts and surveys to interface with service members and their families to identify resources and services to promote availability of EFMP to support individual needs; assisting families in assertively pursuing rights to resources available to them.
  • Participates as assigned in the Inclusion Action Team (IAT).
  • Ensures collection of data in compliance with policies and guidance.
  • Facilitates the development and collaboration of installation EFMP outreach and awareness campaigns to include planning of local events.
  • Participates in coordination and execution of special needs camps, support groups for EFMP families, special events and workshops coordinated by the EFMP.
  • Collaborates with eligible families to coordinate use of the Navy services, such as Respite Care Priority Housing and TRICARE Extended Care Health Option (ECHO).
  • Works in concert with the EFMP team to ensure the program works effectively to support families while maintaining compliance with all legal and regulatory guidance.

B. Training (duties performed 20% of the time include but not limited to):

  • Ensures command EFMP personnel are properly trained to provide briefings to commands that include eligibility, maintaining confidentiality, and available support services.
  • Develops, updates and maintains installation EFMP education, outreach and public affairs programs, to include awareness briefings for command, unit, community organizations, individuals and families and distinguished visitors.
  • Develops and facilitates workshops and trainings on EFMP and other topic of interest to families and commands.
  • Maintains personal records of required and completed staff training as well as, professional development trainings completed to comply with program requirements and to gain knowledge to enhance ability to provide up-to-date and relevant service and support.

C. Case Management (duties performed 60% of the time include but not limited to):

  • Communicates directly with each assigned EFMP family case upon notification of assignment to offer support services.
  • Interviews and assesses individual client and family circumstances upon and during enrollment to identify and evaluate their special needs and situational stressors, determine services for appropriate assistance, provide tailored guidance and updated information on services to improve quality of life and track progress of activity.
  • Maintains a case file in the Navy Accountability and Assessment System (NFAAS) for each assigned EFMP family that includes the Installation Support Privacy Consent Form and detailed contact notes.
  • Documents and maintains current records in the Fleet and Family Support Management Information System (FFSMIS), as required.
  • Assists service members and their families with accessing available resources and services to support individual needs.
  • Provides guidance to families regarding identifying early intervention and special education needs. Helps develop strategies to meet the identified needs and assists with navigating the processes needed to connect them with helping agencies, programs, and resources within the community.

In addition to the educational requirements outlined in the "Education" section of this announcement, candidates must also possess one year of experience working in a case management capacity with families who have members with special needs. The following Knowledge, Skills and Abilities are required for this position:

  • Knowledge of the dynamics of Exceptional Family Member Program and local community resources for families enrolled in the program.
  • Knowledge and understanding of requirements and operations of DoD and Department of the Navy (DON) Exceptional Family Member Program.
  • Ability to work with military families.
  • Knowledge of SECNAV and OPNAV policies and procedures.
  • Ability and skillset to provide effective trainings and briefings.
  • Ability to identify needs, trends, and performance measures to evaluate program effectiveness and provide recommendations for process improvements.
  • Knowledge of military community support programs.
  • Knowledge of federal and agency regulations, the specific organization's programs and missions, pertinent laws, regulations, and policies related to program/operations area in order to consider their impact on programs.
  • Ability to effectively communicate both orally and in writing clearly, concisely, and effectively.
  • Ability to analyze issues that identify best courses of action, determine potential organizational impacts, maintain awareness of barriers impeding mission accomplishment and recommend corrective actions.
  • Ability to develop trusting helping relationships and work effectively with individuals and families from diverse racial, ethnic, religious, and socioeconomic backgrounds.

 

Education

  • A degree in behavioral or social science or related field appropriate to the position.

OR

  • A combination of education and experience that provided knowledge of one or more of the behavioral or social sciences equivalent to a major in the field.

OR

  • Four years of appropriate experience that demonstrated acquired knowledge of one more or more of the behavioral or social sciences equivalent to a major in the field.
