Licensed Social Worker

We are searching for a Licensed Social Worker to work full time (35 hours per week) in our Senior & Adult Services program Monday through Friday. The Licensed Social Worker (LSW) supports our Senior and Adult Services program, which ensures health equity for older adults in Summit County through wellness, prevention, healthcare access, and safety initiatives. As an LSW, you will assess, plan, and implement strategies to enhance the health and independence of our senior community members. Key Responsibilities:
- Provide case management through programs through our GateKeeper program, assessing the needs of older adults to support safe, independent living.
- Providing BRI Care consultations, a caregiver support program supporting individuals with dementia and their caregivers.
- Facilitate community education on brain health, falls prevention, and dementia capability, collaborating with programs like Dementia Friends Ohio to promote an age-friendly Summit County.
- Conduct outreach for caregivers, focusing on communication, self-care, and resources.
- Collaborate with community groups and partners, administer grants, and ensure timely and accurate documentation.
Requirements:
- Bachelor's degree in Social Work or related field.
- Active LSW license in Ohio, with at least 3 years of relevant experience.
- Familiarity with local resources for seniors and experience working with multi-need families.
- Valid Ohio driver's license, automobile availability, and insurance coverage.
