PAY AND BENEFITS

Annual Pay Range: $53,767.31 - $67,749.92

Hourly Pay Range: $27.572979 - $34.743549

Salary offers will be made within the posted pay range and will be based on a candidate's experience (paid or unpaid) that is directly relevant to the position.

JOB DETAILS AND QUALIFICATIONS

Clackamas County's Social Services Division is currently seeking a Veteran Outreach Case Manager to provide support and outreach to veterans experiencing housing instability and at risk of eviction. This role will focus on veteran outreach, working with community partners to enhance housing stability through collaboration and engagement. As a Veteran Outreach Case Manager, key duties will include eligibility determinations, housing screenings, placements, and retention efforts. All services will be delivered in a culturally responsive manner, with a focus on military culture and the specific needs of veterans in Clackamas County.

The ideal candidate will have a demonstrated understanding of person-centered, Housing First approach to case management and knowledge of veteran needs and advocacy. Candidates with military experience and/or veteran status are strongly encouraged to apply.

Required Minimum Qualifications/ Transferrable Skills:*

• A minimum of two (2) years of relevant experience that would provide the required knowledge and skills to perform the duties of the role
• Direct case management experience, outreach experience, or a comparable function, working with homeless and/or low-income populations
• At least one (1) year using creative problem solving, with successful results, in preventing people at high risk of homelessness from becoming homeless or helping people who have been homeless access and maintain permanent housing
• Experience building relationships and networks with local landlords, program participants, colleagues and community partners
• Experience using Microsoft Office products including Outlook, Excel, and Word
• Experience using virtual programs such as Zoom and Microsoft Teams
• Knowledge of veteran needs and advocacy gained through education, training, work experience, veteran status, or lived experience
• Demonstrated commitment and experience advancing equity and diversity in a workplace or community setting

Preferred Special Qualifications/ Transferrable Skills:*

• Bilingual fluency in English and Spanish
• Knowledge of social, health care, culturally specific, financial, and general community resources
• Familiarity with Service Point Homeless Management Information System

TYPICAL TASKS

Duties may include but are not limited to the following:

• Provide homelessness outreach and eviction prevention services to veteran households using a person-centered, Housing First approach. Coordinate placements through the Veterans By-Name List, supporting housing retention, and enhancing housing stability through outreach and engagement. Emphasize collaboration and culturally responsive services tailored to the unique needs of veterans.
• Assess and document program eligibility for cases managed by the veterans housing team, while overseeing the veterans outreach email and phone line. Accept referrals from the Coordinated Housing Access system and internal or external sources within Clackamas County. Gather necessary documentation and information to verify initial eligibility for residents, ensuring compliance with various funding requirements.
• Promote and advance equity in all aspects of work with veteran populations, including case management, support for team members, and both initial and ongoing engagement. Ensure an equitable approach in outreach and services to veterans and within the broader community as appropriate.
• Establish participant rent, utility, and eligible arrears payment levels based on participant income, documented outstanding unit rent and federal, state, and grant requirements. Prepare initial and subsequent rent and related payment vouchers in compliance with state and county regulations.
• Work closely with participants to access needed services and transition back to paying full rent on their own, including a household budget if appropriate. Help address participant needs, preferences, and values using assessment and communication skills. Assist participants to receive additional needed and preferred services using knowledge of public and private resources.
• Coordinate with veteran-specific health care provides, social service personnel, and other agencies to help veteran participants access needed services, including but not limited to employment and training programs, VA benefits, unemployment, SNAP, TANF, and health care enrollment. Advocate for veterans as needed and requested with public and private agencies, schools, and other relevant organizations.
• Assist the supervisor and manager in the Homeless Veterans Coordination Team. Collaborate closely with the unit supervisor to manage and distribute the Veterans By-Name List. Maintain a current record of service providers supporting the veteran community in Clackamas County. Represent Clackamas County Social Services Division in veteran-focused activities with both internal and external networks as needed and requested.
• Collect and submit required data for Service Point Homeless Management Information System at entry, exit, and in the event of interim changes while also tracking certain data needed for program improvement and equitable service delivery.
• Proactively prevent emergencies for veteran participants through clear and consistent communication with landlords and property managers. Respond to emergencies involving veterans and work to negotiate and resolve issues promptly.
• Report witnessed or suspected abuse and neglect of children, older adults, and/or people with disabilities.
• Other duties as assigned from Department of Health, Housing, and Human Services leadership.

WORK SCHEDULE

This position is included in the County's alternate workweek program, working 37.5 hours during a standard workweek of Monday through Thursday (Fridays off):

• Specific hours of work will be discussed with the candidate selected for this position at the time an offer of employment is extended.

The exact amount of on-site/telework and schedule will be discussed with the hiring manager during time of offer. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.
