SUMMARY: This position is responsible for providing comprehensive services to individuals experiencing chronic homelessness, and who may also be living with addictions and mental illness. The case manager collaborates with program participants to ensure long term housing stability and connections to community resources. Successful candidates are committed to ending homelessness and dedicated to CCC's mission of serving the most vulnerable in our community, including older adults, individuals with disabilities, LGBTQIA identifying individuals, and those traumatized by systemic inequities.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
Provide housing-focused case management to households participating in the services offered by Supportive Housing Program.
Prepares and maintains complete and accurate case files and case records.
Creates housing stabilization service plans collaboratively with program participants and ensures progress towards the participant’s housing and financial goals.
Coordinate services with appropriate community agencies to address housing barriers and complex needs of participants.
Assist participants with developing long-term permanent housing goals and strengthening community networks.
Coordinates with local social service agencies, health departments, and medical centers to ensure program participants are receiving services necessary to achieve personal goals.
Conducts annual assessments and re-certifications to ensure households meet eligibility criteria and are progressing on service plan goals.
Participates in staff trainings. May facilitate trainings to staff and volunteers, as assigned.
Assists in developing community partnerships and resources.
Collaborates with landlords and other housing liaisons to seek out cost effective rental opportunities for clients.
Negotiates with landlords and other housing liaisons to reduce fees and/or avoid evictions.
Ensures completion of appropriate safety inspections for client housing opportunities.
Refers clients to other services and programs as needed.
Enter all appropriate information into agency database in a timely manner.
Processes check requests for rental and utility assistance with the finance department according to funding guidelines and program requirements.
Creates and distributes all required reports in a timely manner and as directed by management.
Represents the Agency in the community and workplace in a professional and ethical manner.
Demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.
Demonstrates strong interpersonal, decision-making, problem-solving, oral, and written skills.
Utilizes a basic knowledge of Microsoft Office applications.
EDUCATION and/or EXPERIENCE: Bachelor’s Degree required with two years of related experience or equivalent education and experience. Must have a valid drivers license
