Social Worker- Community Support Program

The purpose of the Social Worker- Community Support Program is to provide social work services to persons with mental illnesses living in the community for the Sauk County Department of Human Services.

 

Essential Duties

 

  • Teach clients in transition from hospital, institution, or assisted living the apartment application process.
  • Provide clients with supportive psychotherapy, symptom management support, and life skills education to allow consumers to attain and maintain their highest level of independent living.
  • Accompanying clients to medical and dental appointments, and other appointments.
  • Provide psychosocial rehabilitation activities and lead social and recreational events and work to improve skills for interaction with neighbors, landlords, doctors, family, and peers.
  • Perform case management tasks such as establishing client budgets and monitoring finances, assessing clients’ needs and referring to services/agencies when necessary.
  • Compile an in-depth assessment for new consumers and perform annual mental health functional screen on current clients.
  • Assist with medication observation, which may include weekends and evenings.
  • Prepare recovery plans and progress notes.
  • Occasional after-hours interventions for CSP consumers as needed.
  • And other duties as assigned.


Required Working Hours
Standard working hours are Monday through Friday, 8:00 am until 4:30 pm.  Please note these hours are subject to change and additional hours may be needed or required.  Compensation for additional hours beyond normal working hours are subject to applicable state and federal regulations.

 

Minimum Training and Education

 

Required:

Bachelor’s degree in Social Work or Behavior Science AND

1000 hours of supervised post-degree clinical experience with chronically mentally ill persons

   OR
 

Bachelor’s degree in another related field AND 

2000 hours of supervised post-degree clinical experience with chronically mentally ill persons

 

Licenses/Certifications: 

Wisconsin State Motor Vehicle Driver’s License

 

Preferred:

Wisconsin State Social Work License obtained

 

Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered.
