Case Manager/Social Worker

We are seeking compassionate and dedicated Case Managers/Social Workers to join our dynamic team. The Case Manager/Social Worker will play a crucial role in developing person-centered support plans, connecting individuals with appropriate services, and advocating for their independence and right to take risks. The successful candidate will ensure compliance with local, state, and federal policies while maintaining accurate documentation of all activities and case notes.
Responsibilities:
- Develop person-centered support plans in collaboration with individuals, parents, and legal guardians.
- Connect individuals with providers and services that align with their unique needs.
- Regularly communicate with individuals, parents, and legal guardians to assess and address evolving needs.
- Ensure services provided by providers comply with individual requirements and adhere to local, state, or federal policies.
- Adjust support plans as needed due to internal or external factors influencing the individual's needs.
- Advocate for the individual's needs, wants, and independence, including the right to take risks.
Qualifications:
- Accredited four-year degree in social work, special education, psychology, nursing, human services, or related fields.
- Or an accredited four-year degree with a major in any field and one year of experience as a social worker/case manager/care coordinator in a public or private social service agency.
- Effective written and verbal communication skills.
- Self-motivated and driven.
- Ability to work with individuals with challenging behaviors.
- Strong organizational and time management skills.
- Intermediate computer and typing skills for accurate case note documentation.
- Problem-solving skills and ability to multitask.
- Capacity to work independently and collaboratively within a team.
- Quick learner, adaptable to a constantly changing environment.
- Ability to handle challenges with a caring and compassionate approach.
- Possession of a Driver's License and car insurance, as the role requires client visits.
- Successful clearance of DHS background check.
Preferred Qualifications:
- Bilingual proficiency.
- Familiarity with waiver programs and services.
- Additional one year of experience in education or treatment of people with developmental disabilities or related conditions, or completion of a relevant course.
Schedule:
- Monday through Friday, 8:30 am to 5 pm.
- Mandatory office work during onboarding.
- Case Manager will eventually transition to up to 4 days a week remotely based on performance.
Sign On Bonus:
- $1000 paid over a period of 6 months.
Job Type: Full-time
Salary: From $23.00 - $26 per hour based on relevant experience.
Expected hours: 40 per week
