Care Coordinator Intern - Blue Cross of Idaho

Overview of Organization: Blue Cross of Idaho is the largest health insurance carrier in Idaho.  This role would be working within our healthcare operations team working with clinical and non-clinical staff that worked directly with Medicare and Medicaid memberships.

Responsibilities:

  • As an integrated member of the Care Coordination team, this role will be working directly with Dual Eligible members, both in-person at the member’s place of residence and telephonically. You will be accompanied by a mentor for these visits. 
  •  Intern will complete primary care coordination responsibilities, including, but not limited to:
    • Coordinate care by working with doctors, healthcare providers and family members
    • Will contact members after enrollment to discuss individual healthcare needs and goals
    •  Will serve as primary contact for coordinating member’s care
    • Assist in scheduling health plan services
    • Complete member outreach and engagement ,
    • Complete member assessments,
    • Develop individualized member Care Plans
    • EMR documentation,
    • Provide member education
    • Participation in internal team meetings in Interdisciplinary Care Team discussions

Qualifications / Applicant Requirements:

  • Strong interpersonal and communication skills; 
  • problem-solving capabilities involving sound, judgment and discretion; 
  • strong written and verbal communication skills, including attention to detail and accuracy; 
  • intermediate technology skills, including Microsoft platforms and electronic health, record system, 
  • valid ID driver license, 
  • Liability insurance
  • reliable transportation. 

Skills intern will develop:

  • Bio-psycho-social assessment; 
  • care plan development;
  • interdisciplinary team collaboration; 
  • Medicare and Medicaid health care system navigation case management; 
  • quality metrics within the healthcare setting.
