Case Manager

Your Impact

 

The Case Manager provides navigation and case management services to clients in need of assistance and recovery support services.  The Case Manager engages clients to create case plans, monitors progress, removes barriers, and provides referral services and linkage to other supports as needed.

 

 

 

Your Skills and Duties

     * Work in coordination with family and supervisor with developing and implementing a case plan for continued services.

     * Maintain a supportive relationship with individual and family to promote progress and achievement of case plan goals.

     * Participate in case plan reviews, program staff meetings, county meetings, and supervision meetings with collaborative partners (i.e. education, employment, counseling) as required

     * Transfer and link client to new service provider(s) to facilitate access to services, benefits and programs to aid in recovery and improve overall health and well-being.

     * Provide support and refer clients to recovery support services such as public benefits, employment counseling, job training, housing stability support, educational resources, food and nutrition programs, wellness programs, behavioral health services, digital literacy training, and etc.

     * Participate in and/or conduct training, support groups or activities.

     * Provide transportation when necessary for activities and appointments.

     * Conduct outreach and engagement activities to inform the community of the services available from the Agency.

     * Maintain accurate and timely client records and documentation.

     * Complete administrative paperwork (time sheets, mileage, time off requests, phone bills & other expense reimbursements, weekly schedule, etc.) on time.

     * Participate in individual weekly case review, individual and group supervision, staff meetings and special events.

     * Perform additional duties as assigned.

 

 

 

Education and Work Experience 

     * BA in Social Work or related field or AA degree and related work experience.

     * Minimum of three years experience in working with individual/families and providing case management experience.

     * Meets all state required conditions of employment as set forth by Community Care Licensing for a Licensed Community Care facility; i.e. Fingerprint and child Abuse Index clearance, TB/Health Physical, valid CDL and clean driving record.
