Promising Practices Home Visitor at The Family Partnership

For more than 140 years, The Family Partnership has built strong families, vital communities, and better futures for children. We fulfill our mission through counseling, education, and advocacy, primarily in the greater Minneapolis and St. Paul area. The Family Partnership was founded in 1878 as the Minneapolis Humane Society to protect and serve children that were being abused or exploited. Along with a few name changes, our work has continually transformed to anticipate and address the evolving needs of families and individuals, many of whom are the most vulnerable in our community.

Wages and Benefits

$24.00 per hour; Generous paid time off, plus 10 paid holidays; competitive health, dental, and vision insurance; health savings account (HSA), and options for flexible spending accounts (FSA), employer paid basic life and accidental death and dismemberment (AD&D) insurance with the option for additional coverage, and employer paid short and long-term disability insurance; agency sponsored retirement plan with a tiered vesting schedule based on years of service, plus a 2% employer contribution after one year of service.

 

SCHEDULE

This is a hybrid position that is mostly virtual but will require weekly travel to clients’ homes and may require occasional in-office meetings or work.

 

A Typical Day in the Life…

A typical day as The Family Partnership’s Promising Practices Home Visitor provides an opportunity to support TFP’s Promising Practices program in buffering young children and their caregivers from the impact of adverse childhood experiences (ACEs) and toxic stress. Responsibilities occur primarily in family homes and will include the following:

  • Working with TFP preschool family workers to identify preschool families enrolled and newly enrolled with a child 3-5 years old who qualifies for Promising Practices.
  • Meeting with families in their homes weekly for at least one hour at each visit.
  • Establishing a trusting relationship with families and using a trauma-informed approach.
  • Helping caregivers identify personal strengths and set personal and family goals.
  • Assisting caregivers with learning and using internal state language and supporting their preschoolers in building executive functioning skills through weekly activity modules and children’s books.
  • Providing strength-based, family-centered interventions and prevention strategies during weekly home visits.
  • Assisting caregivers with identifying ways to increase monthly income.
  • Helping parents improve skills to optimize their home environments and family support systems.
  • Supporting families using two-generation approaches to put them on a path towards economic security.
  • Referring, advocating for, and following up with clients in need of additional community support and services.
  • Documenting all interactions, including case notes, assessments, intakes, surveys, discharge summaries, transfer summaries, and evaluations.

 

Who You Are

  • You hold a bachelor’s degree in human services, social work, education, or a related field.
  • You have a minimum of two years of directly related work experience.
  • You have previous experience working with families in homes and/or community settings.
  • You have previous experience working with and understanding communities living in poverty, communities of color, gay, lesbian, bisexual, and transgender individuals, and other vulnerable populations.
  • You possess excellent verbal and written communication skills to effectively engage families and document the work properly.
  • You have the ability to travel to clients’ homes, schools, meetings with community partners, and various other locations and are willing to transport clients in your personal vehicle.
  • You have the ability to develop and maintain positive and respectful relationships with clients.
  • You have the ability to track and maintain data necessary for program monitoring and evaluation.
  • You have the ability to perform assessments needed to conduct appropriate interventions and referrals.
  • You have the ability to work respectfully and communicate effectively with different cultures, groups, and languages.
  • You have the ability to work some evenings as required.
  • You have a valid driver’s license, access to reliable transportation, and meet TFP’s driving record requirements. If your license was not issued in the state of Minnesota, you have 60 days after becoming a Minnesota resident to apply for a Minnesota driver's license or permit (not applicable for new hires living out of state).
