Behavioral Health Case Manager

Job Summary:
Primary responsibility is to provide community support services to patients with significant mental illness by coordinating care and providing life skills training that appropriately meets their needs and reduces barriers to health care services and daily life struggles that will help the patient to further optimize their overall well-being.

 

Primary Duties & Responsibilities:

  • Manage a Community Support Services caseload providing coordination of care and daily living skills training as medically necessary as a higher level of care to patients in branch offices, in the community, and in their homes as needed, individually and in group settings.
    • Including scheduled appointments, development and monitoring of treatment plans, and documentation completion;
    • Including running Choices groups for daily living skills;
    • Including transporting patients in a company car as appropriate.
  • Manage a caseload within the Meds Only Program as a lower level of care to patients taking psychotropic medication, including scheduled appointments, development and monitoring of treatment plans, and documentation completion.
  • Maintain a caseload within the Restoration Program, including scheduled appointments and documentation completion.
  • Maintain appropriate productivity rate monthly.
  • Collaborate with clinical staff in the development and implementation of the plan of care and achievement of goals.
  • Shall attend team meetings in order to facilitate an integrated, multidisciplinary team approach to services:
    • Individual supervision for review of recent contact with patients and review of documentation;
    • Clinical supervision meetings, as appropriate, for training and education.
  • Provide coverage at the Regional Assessment Center on a rotational basis when needed, including outside business hours.
  • Go on secure transports, in a company car, when needed.

Knowledge and Skills:

  • Must be able to produce work with a high degree of accuracy and attention to detail with a history of demonstrating efficient multitasking skills and prioritization of work assignments.
  • Must be familiar with community agencies and have the ability to work independently with internal & external customers.
  • Have a core philosophy or values consistent with a community-centered approach to care.
  • Demonstrate a sensitivity and responsiveness to varying cultural characteristics and beliefs and have a spirit of kindness and concern.
  • Maintain professionalism and positive attitude.
  • Bilingual (Spanish/English) is preferred, but not necessary.

Education and Experience:

Bachelor's degree in social work, counseling, psychology, or related field from an accredited institution. Applicants with a bachelor’s degree or higher in a non-related field may be considered if they have one year in the health field.
