Hourly Range: $28.58 - $38.57

Who We Are:
Mental Health Support Services (MHSS) is the provider of mental health, developmental disabilities, and substance use disorder services for children and adults in Chesterfield County. We are currently seeking a PT Service Coordinator who has a passion for working with kids and adults with intellectual and developmental disabilities reach their desired goals.
 
Minimum Qualifications:
Bachelor's degree in a human services field, such as psychology, sociology, rehabilitation counseling, or a related field; two years of experience in a human services field; or an equivalent combination of training and experience. Two (2) years of direct service to those with Intellectual Disability (ID) Developmental Disability (DD) diagnosis preferred.
 
Required Knowledge, Skills, and Abilities:
Working knowledge of case management methods, practices, and procedures. Ability to interview clients to identify deficiencies in personal, social, economic and health needs; to work in a multi-disciplinary team environment providing treatment and rehabilitation services; to communicate effectively orally and in writing; to operate a personal computer and related software and other standard office equipment; to develop and maintain effective working relationships with internal and external customers. Knowledge of Intellectual Disability (ID) Developmental Disability (DD) Services in VA to include the Developmental Disability (DD) Waiver System preferred.
 
Additional Requirements:
Current valid driver’s license and good driving record required. Based on the Virginia DMV point system, record must not reflect a total of six or more demerit points within the twenty-four months preceding the anticipated hire date, or one major violation of six demerit points within the preceding thirty-six months. Out of state driving records must be obtained by applicant and presented at time of interview. Record must reflect at least three years of history and be dated within thirty days of interview date.

Pre-employment drug testing, FBI criminal background check, and education/degree verification required.

This position is considered Safety Sensitive and is subject to random drug and alcohol testing.
 

Duties include but are not limited to the following:
 
 • Interviews patients/clients and their family members to obtain pertinent social, economic, occupational, and physical and mental health
 information;
 • Connects to and monitors community services for kids and adults with intellectual and developmental disabilities;
 • Collaborates with community partners for advocacy for individuals with disabilities;
 • Monitors services provided by the DD Waivers;
 • Educates clients and their families on their legal rights and provides information on department policies, procedures, and services;
 • Prepares and maintains casework documentation as required in the delivery of services (e.g. case histories, assessments, treatment plans, progress notes, correspondence) and;
 • Performs other work as required.
