Oaklawn is the only faith-based provider for comprehensive mental health and addictions services for children, adolescents, adults and seniors in St. Joseph and Elkhart Counties in Northern Indiana. What You’ll Do
Case Managers help youth and families in our community live healthier and more fulfilling lives through identifying and supporting their needs and goals. Case Managers specialize in:
1. managing the care provided to youth and families, and
2. helping youth and families gain access to the services they need to meet their goals
Primary duties include:
- Evaluating the family’s need for services
- Developing and monitoring a treatment plan that supports the family’s goals
- Helping the family obtain services needed to support their goals, such as food or housing assistance, medical care, Social Security benefits, transportation, skills training, etc.
- Following up to make sure the family is receiving services and those services are helping
- Advocating for the family and serving as a link between DCS, court, schools, healthcare providers, employers, community agencies and other Oaklawn services
