Position Summary
The Physical Therapist is responsible for evaluating, planning, directing, and administering of Physical Therapy services as prescribed by a licensed Provider. Treatments provided are to restore function, improve mobility, relieve pain, prevent/limit physical disabilities and/or increase independence to improve patients' overall functioning within their environment.
General Duties, Tasks and Responsibilities
- Treats patients with Physical Therapy modalities (ex. Ultrasound, NMES, iontophoresis, etc.), skilled therapy techniques (ex. Manual muscle testing, joint ROM measurements, etc.); instructs patients on use of adaptive/assistive equipment.
- Supervises and directs Physical Therapy Assistants (PTA); maintains open communication with members of the Rehab team and consults other departments as appropriate to work collaboratively toward positive clinical outcomes.
- Performs an appropriate assessment on all patients as related to physician order; interprets results within scope of licensure.
- Designs treatment programs for patients of varying degrees/types of disabilities and multiple medical problems in accordance with accepted practice patterns and physician orders.
- Formulates realistic and appropriate short and long term treatment goals for patients. Includes patient/family in development of the plan of care.
- Observes and evaluates treatment effect, recommending changes to physician as necessary.
- Ensures that patient charges and documentation are accurate, entered on a timely basis and in compliance with regulatory standards.
- Provides information related to the departmental budget in relationship to Physical Therapy needs for optimum patient care and service.
- Documentation is timely, efficient and meets current standards and policies; demonstrates thorough knowledge of computerized documentation including: evaluations, progress notes, monthly notes and discharge summaries.
- Performs all aspects of the position in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.
- Participates in performance improvement activities.
- Performs other duties as assigned.
Education Requirements
- Current Professional Licensure Required
Certification/Licensure Requirements
- Current IL Physical Therapist License Required
- Valid Driver's license and proof of personal auto insurance Required
- BLS Required
- APTA Membership Preferred
Experience Requirements
- Physical Therapy minimum 1 year Preferred
Computer Skills
- Strong computer skills including Microsoft Word, Excel and Outlook
