Social Needs Navigator

The Greater Richmond YMCA is currently seeking full-time positive, encouraging, and dedicated individuals to work as Social Needs Navigators with our chronic disease and social health programs. The Social Needs Navigator works in collaboration with internal YMCA team members, and external members of the client's care team. The Navigator accepts referrals and manages assigned caseload, while developing care initiatives to assess and coordinate care management for their client. The Navigator serves as an essential link, guiding and supporting clients through and around barriers of lifestyle change.

Duties of a Social Needs Navigator:

The Social Needs Navigator will be responsible for connecting individuals to services that address basic services including, but not limited to, housing, healthcare, food, as well as community health programming within the YMCA and offerings within the community. The Navigator will conduct in person, or small group coaching as scheduled. Candidates must be able to work occasional evenings and weekends.

Basic Qualifications of a Social Needs Navigator:

  • At least 18 years of age
  • Bachelor's degree or certified community health worker
  • Experience working in behavior change, wellness, specialized populations or social work
  • Must possess health coaching skills and Listen First/Motivational Interviewing skills
  • Ability to travel to YMCAs within the Metro area.
  • Proficiency in Microsoft suite and ability to multi-task on various digital platforms
  • Must comply with organizational compliance and privacy policies.

Preferred:

  • Experience in case management or social work
  • Skills in motivational interviewing/health coaching
  • Experience in data collection and entry
  • Bilingual candidates preferred.
  • Community Health Worker preferred.
  • Social Work License/Certification preferred.

Required:

  • Transportation - this position will require in-person engagement at multiple locations
  • CPR/AED/First Aid certification required upon hire

Salary: $48,000 - $50,000
